The Southwestern Connecticut Agency on Aging (SWCAA) is a private, not-for-profit corporation, established in 1974. As a designated Area Agency on Aging, according to the Older Americans Act of 1965, as amended, SWCAA is a funding source for federal and state dollars for the elderly in the fourteen town region of southwestern Connecticut. 
 
Funding supports services such as nutrition, in-home, legal, health, adult day care/respite, transportation, senior centers and outreach/social support. SWCAA researches and evaluates elderly issues, offers community education related to the needs of the elderly, and serves as an advocate for older individuals. It is a resource for information and referral to services for older adults, such as health insurance, housing, and in-home care. The Agency administers the Statewide Respite Program and the National Family Caregiver Support Program for the region. SWCAA is also an “Access Agency”, under contract with the State of Connecticut, Department of Social Services, to provide care management to clients who receive home and community-based services through the Connecticut Home Care Program for Elders.

Provides up to $3,500 in services to caregivers caring for a loved one with Alzheimer’s or Dementia.  This program can pay for in-home help, adult day care, meals on wheels, in-patient respite to allow the caregiver to take a needed vacation and other services. 
 
 
Provides up to $3,500 to caregivers caring for a loved one over the age of 60.  This program also can pay for in-home help such as a home health aide, companion and homemakers as well as adult day care, in-patient respite, emergency response systems and home-delivered meals.
 
 
CHCPE is a client-centered home care program which is designed to enable individuals at risk for institutionalization the option of remaining in the community safely in the least restrictive environment possible. The Southwestern CT Agency on Aging (SWCAA) administers CHCPE through the Care Management Department. Services are funded through the Department of Social Services (DSS) and Medicaid. To be eligible, a client must be a CT resident, 65 years of age or older, at risk for Nursing Home Placement, and meet financial and functional eligibility criteria. After a referral to DSS, a qualified Care Manager will perform an Initial Assessment. Final eligibility is determined by the Department of Social Services (DSS). There are three levels of services available in CHCPE. Your Care Manager will help to determine the appropriate service level for each person based on their eligibility.
