Byron Cordes, LCSW, C-ASWCM

President/ Care Manager

Byron Cordes received a Masters in Social Work from the Worden School of Social Services at Our Lady of the Lake University and a Bachelor of Science in Psychology from Oklahoma State University. He has worked for over 20 years in the social work field. His background includes medical social work/case management and clinical work with geriatrics. Byron is a Licensed Clinical Social Worker in the state of Texas. He also holds an Advanced Certification in Social Work Case Management from the National Association of Social Workers. He has served as an Assistant Professor at Our Lady of the Lake University in San Antonio, and continues to teach, as adjunct faculty. Byron has had his writings published on geriatrics and caregiving. He currently owns and operates Sage Care Management, a Professional Geriatric Care Management agency, providing case management, outreach and support to seniors. He serves on the National Board of the National Association of Professional Geriatric Care Managers and is past-president for the South Central Chapter. He is a member of the American Society on Aging and the National Association of Social Workers. Byron has been married for twenty-one years, and has one daughter. He is active with several local community organizations including his church.

Nancy Ortiz, RN, CMC

Care Manager 

Nancy is an experienced geriatric care manager with more than 20 years of nursing experience. She has been working as a geriatric care manager for the past 8 years. Other nursing experience includes working with hospice patients and their families, nursing in a hospital setting, medical case management and working several years with the special needs population. She is a member of the National Association of Professional Geriatric Care Managers and the National Geriatric Nurses Association. She is a Care Manager certified through the National Academy of Certified Care Managers. Nancy is also a volunteer court visitor with the Guardianship Program through Catholic Charities and is an active member of the local Task Force Against Elder Abuse. She was graduated from Baptist Hospital School of Nursing in San Antonio. A little known fact is that she is an avid and passionate collector of buttons.

Kimberly Haney, LCSW, C-ASWCM

Care Manager

Kimberly is a graduate of Our Lady of the Lake University with a Masters in Social Work. She is a Licensed Clinical Social Worker in the state of Texas. Kimberly has worked extensively with psychiatric disorders and in case management. Kimberly is a Licensed Clinical Social Worker in the state of Texas. She also holds an Advanced Certification in Social Work Case Management from the National Association of Social Workers.

Shawn Loup, BSN, RN

Care Manager  

Shawn Loup has been with Sage nearly since the beginning. She has experience with critical care nursing at the South Texas Audie L. Murphy Veterans Hospital and also works part-time as a school nurse. She is married with one child.

Ann Pruski

Office Manager & Care Manager Assistant

Ann joined our team in March 2010. She comes to Sage Care Management with more than 15 years Human Resource Management experience. Ann holds a BA in sociology from the University of Texas at San Antonio with a specific interest in geriatric care. Ann is married with two children and enjoys spending her free time fishing in Rockport with her family.

Natalie Cordes, BS

Care Manager Assistant   

Natalie has a special talent for organization. She is able to help clients downsize or just get their current home in shape. More importantly, she keeps Byron in line (yes, she is his wife). Natalie holds a bachelors in psychology from Oklahoma State University. She enjoys time with her family and creating jewelry.

Services

How We Help:

Whether you have family at a distance or care for someone in town, Sage Care Management can help. Our professional staff are trained in issues facing seniors and their caregivers. We employ Licensed Master Social Workers and Registered Nurses to provide on-site assessment and case management.

Sage Care Management can:

  • Conduct personalized care-planning assessments to identify problems, eligibility for assistance, and need for services
  • Screen, arrange and monitor in-home help or other services
  • Review financial, legal, or medical issues and offer referrals to geriatric specialists to avoid future problems and conserve assets
  • Provide crisis intervention
  • Act as a liaison to families at a distance, making sure things are going well and alerting families to problems
  • Assist with moving to or from a retirement complex, care home, or nursing home
  • Provide consumer education and advocacy
  • Offer counseling and support
