About Us

Lake Cook Health & Rehab Center is located in the quiet suburban setting of Northbrook, Illinois. We are only thirty minutes away from Downtown Chicago, and forty five minutes from the Wisconsin border. Lake Cook Health & Rehab Center is located just off Eden's Expressway in the Northbrook Village Square Mall.

Lake Cook Health & Rehab Center has been providing service to our community for over 40 years. Our commitment to excellence guarantees the most professional, yet caring atmosphere each individual requires.

With a modern perspective, Lake Cook Health & Rehab Center offers quality care to all those residing within our facility. Our goal is not only to heal the physical but the spiritual person. Skilled Nursing

Lake Cook Health & Rehab Center provides skilled and intermediate nursing care twenty-four hour a day for each Residents individual health care needs. Our interdisciplinary team meets daily to monitor our Residents progress.

    Our facility has recently been accepted into the Illinois Dept. of Public Aid Exceptional Care Program. This program offers skilled care services to those Residents who do not qualify for Medicare.

Rehabilitation

Lake Cook Health & Rehab Center provides all our Residents with the necessary therapy services to attain and maintain the highest level of functioning possible. With our aggressive 7 days a week therapy program our Residents have the opportunity to receive Physical, Occupational and Speech therapy We offer Activities of Daily Living Training (ADL) to re-teach the lost skills of self-care, work and leisure activities.
