Deaconess HomeCare‘s broad spectrum of effective and efficient home health care services and programs are provided by highly-trained professional staff who utilizes state of the art technology. An individualized comprehensive plan of care, addressing the total patient need, is created in conjunction with the patient’s physician. Staff is available 24 hours a day to respond to questions, make suggestions and swiftly determine a course of action.

Providing home care since 1969, DHC received the “Nurse Employer of the Year” award in 2008 along with accreditation status by the Accreditation Commission for Health Care, Inc. Who is appropriate for home care?
Those who are disabled, acute or chronically ill, have a life-limiting illness, or are recovering from surgery.

An evaluation will be done by a nurse or therapist to assess the specific needs. A plan of care is developed with the MD, DHC team and family to determine the best course of treatment for the patient.

- Changes in ability to walk
