Since 1969, Deaconess HomeCare has offered both experience and a commitment to providing quality services that enhance the well-being of the patients and the communities we serve. Deaconess HomeCare‘s broad spectrum of effective and efficient home health care services and programs are provided by highly-trained professional staff who utilizes state of the art technology. An individualized comprehensive plan of care, addressing the total patient need, is created in conjunction with the patient’s physician. Staff is available 24 hours a day to respond to questions, make suggestions and swiftly determine a course of action.

Speciality Programs
Deaconess HomeCare’s (DHC) carefully screened and trained staff will care for you or your family member in the comfort of your own home. Providing home care since 1969, DHC received the “Nurse Employer of the Year” award in 2008 along with accreditation status by the Accreditation Commission for Health Care, Inc. (ACHC) for the services of home health and hospice. DHC serves 89 counties through the operation of 24 locations with staff available to you 24 hours per day.
Who is appropriate for home care?
Those who are disabled, acute or chronically ill, have a life-limiting illness, or are recovering from surgery.
Patients in need of:
- Infusion therapy
- Low vision therapy
- Cardiac therapy
- Balance rehabilitation
- Pain management
- Joint therapy
- Speech therapy
- Wound care
- Ostomy or continence management
An evaluation will be done by a nurse or therapist to assess the specific needs. A plan of care is developed with the MD, DHC team and family to determine the best course of treatment for the patient.
Conditions that might indicate a need for home care
- Frequent Emergency Room visits
- Recent weight loss or gain
- Frequent re-hospitalizations
- Changes in ability to walk
- Change in current diagnosis
- Changes in ability to swallow or speak
- Frequent falls or dizziness teaching, assessment and observation
- Vision problems
- Medication changes or new medications
- Decreased independence/that would require teaching self-care capabilities
