About Us
On behalf of the residents and staff of Bloomington Rehabilitation & Health Care Center, I would like to extend a very warm welcome to you and your loved one. The staff of Bloomington Rehabilitation & Health Care Center strives for excellence in obtaining each resident’s individual needs and goals to attain their highest level of independence.
Our main objective is to provide excellent nursing care. We have a very dedicated and caring nursing staff to meet your needs. We also offer an exciting activity and social service department to provide for your social needs. We strongly encourage family involvement with our residents at any time.
Nursing Care
Each of our homes offers a licensed nursing staff with skills above and beyond the requirements of a typical Medicaid skilled nursing facility. Highly educated, our staff is able to meet a variety of health care needs, including rehabilitation, dementia/Alzheimer’s care, and others as may be required at various stages of life.
Once the decision has been made that 24-hour care is necessary and must be provided by a skilled nursing facility rather than assisted care selecting the right home will be an important decision. Unfortunately, this decision is often made in a difficult or crisis situation. If at all possible, it’s important to visit several homes – perhaps more than once – so that you can evaluate whether it will suit the needs of your loved one.

Pathways’ rehabilitation program delivers the highest standard of quality care in the most cost-effective and value-added manner. Individualized treatment programs are designed to serve patients in a wide variety of clinical categories.
Our unique service model offers:
- Physical, occupational and speech therapies available seven days per week.
- An interdisciplinary treatment approach by a team of highly qualified rehabilitation professionals.
- Family involvement in treatment and care planning processes.
- On-site Rehabilitation Program Coordinator to coordinate and manage the efforts of the interdisciplinary rehab team.
- Pre-admission screenings to assess each patient’s appropriateness for the program.
Free local transportation to and from follow-up doctor appointments.

Rehabilitation
Once you have determined whether an assisted living or long-term care facility is required and/or a physician determines the level of care and rehabilitation requirements, you will also need to evaluate what services are offered. Some facilities specialize in rehabilitative care, while others offer only a minimum of service. When you are choosing a facility with rehabilitation, you will need to consider not only the traditional characteristics (see Skilled Nursing), but the following tips may help as well.
