Home Health Care Program
The VNA of Central Connecticut, Inc. provides health care that is delivered to you... at home.
Combining the skills and knowledge of professional and support staff, the VNA provides for your home health needs. Whether you've been recently hospitalized, diagnosed with an illness, or wish to remain in the comfort of your own home while coping with changing health conditions, we can help.
A plan of care specific to your needs and approved by your physician will be developed by you, your family and the VNA health care professional.
What is Hospice?
Hospice is a special kind of care for people and families living with a life threatening illness.
Hospice care is focused on comfort rather than cure. Hospice provides physical, psychological, social and spiritual care to patients and their families.
Hospice homecare is provided on a part time, intermittent, regularly scheduled basis. Also a nurse and physician are available on call 24 hours/day, 7 days/week to provide advice and support and make visits as necessary.
Admission to a Hospice program is based on patient and family need and consent.

CareLink
Seniors and Students: Partners for Wellness
Linking generations together to meet the ongoing health needs of the senior community.
The mission of the CareLink program is to promote individuals and their families' ability for selfcare and to empower them to increase and/or maintain a healthful quality of life. This requires an informed client who is well supported by a network of family, friends and health providers.
One call puts you in touch with the Visiting Nurse Association of Central Connecticut, Inc. and any other community resources you may be looking for.
CareLink Program:
Is a FREE service of the Visiting Nurse Association of Central Connecticut for residents of New Britain, Plainville and Southington who are 60 years and over interested in ongoing health monitoring, education and/or prevention.
Services are provided in the home by nursing students from affiliated schools.
Services Provided:
- Inform seniors of the many programs and services of the VNA of Central Connecticut, Inc. and the community.
- Teach clients how to maintain health in the presence of a chronic disease.
- Promote a higher level of fitness and functioning
- Assist in understanding individual health issues, medical diagnosis, medication, insurance coverage, diet, and locating, appropriate resources.
- Inform clients of health choices and make recommendations on how to address health issues and concerns.
- Provide socialization and emotional support to clients and/or their caregivers.
- Monitor health status on an ongoing basis and assist in making and following up on referrals to appropriate healthcare resources.
- Where appropriate, coordinate with caregivers and/or family on clients' healthcare status/needs.
Activities Include:
- Telephone outreach
- Home assessments
- Health teaching/referrals
- Group teaching with special populations
