Toward that goal, the United Hospice team provides dedicated and quality holistic care, treating the physical, social, and spiritual needs as our patients approach their final days.
Individuals become eligible for our hospice services when a physician certifies that a patient admitted to hospice has a probability of six months or less to live. Eligibility is not a function of the patient’s diagnosis, rather it is a function of the patient’s prognosis.
Once someone becomes a United Hospice patient, he or she receives services as long as the services are required. Services are not discontinued unless they are no longer necessary or the patient chooses to have them ended. United Hospice serves individuals who need and want hospice services at home, with friends or relatives, or in a long-term care facility or hospital. United Hospice coordinates the delivery of all needed services.
United Hospice retains a medical director as a member of the care planning team and as a consultant to the hospice staff. The patient’s own physician continues to direct and manage the patient’s plan of treatment. The care plan is updated as needed, and at least monthly, to reflect changes in the patient’s condition. United Hospice staff visit patients and family as often as necessary. The frequency and focus of our visits is a function of the needs of the patient and the family. Should continuous care be indicated by the patient’s condition, United Hospice meets those requirements.
If you believe someone needs hospice services, and the patient or his or her family wishes to receive hospice services, call United Hospice. With a physician’s order, United Hospice will assess the patient and develop a plan of care for the physician to review.
