TLay Healthcare Services, Inc. will be the preferred home healthcare agency in the provision of safe, timely, and quality home health care by dedicated and caring health care professionals.

TLay Healthcare Services, Inc. was incorporated on April 24, 2006 as a registered home health agency serving Northeast Florida counties- St. Johns, Duval, Clay, Flagler, Baker- with main office located in the historic St. Augustine, Florida.
Dr. Imelda Nwoga is the Administrator of TLay Health Care Services, Inc. since taking ownership in 2007. Striving to be a leading provider of home health care, Imelda has experience providing holistic care to clients and has been influential in planning, developing, and directing the operations at the agency.
Her academic preparation includes a doctorate degree in Community Health Nursing with a minor in Medical Anthropology from the University of Florida. She holds two Masters degrees in Curriculum and Instruction with an emphasis in Nursing Education from the University of Wisconsin and in Community Heath Nursing from D’Youville College. Her Bachelors degree was earned in Nursing from the University of Wisconsin in Madison.
As Administrator, Dr. Nwoga hopes to apply her knowledge and over 25 years of teaching and leadership experience to promote safe, timely, and quality client care at TLay. She would like to be a role model to other health care providers in fostering professionalism and excellence within the health profession.
At TLay we staff dedicated, team orientated and trained professionals that are eager to provide a variety of services such as nursing, physical therapy, occupational therapy, speech therapy, medical social work, and case management according to the client’s plan of care.
