Welcome to The Ponce, a skilled nursing facility serving St. Johns County and the surrounding areas.

Making the decision to place a loved one in a skilled nursing facility is difficult, and most do not consider it until the decision has to be made. Our hope is that we can help make your decision a little easier by providing a place where your loved one will be cared for as they should be - as a mom, a dad, a husband, a wife, a brother, a sister, a grandpa, a grandma.

We believe that quality care begins and ends with people. Every resident is special, which means what we do must be special. We're not caring for room numbers or charts. We're caring for people. Services

Therapy

While The Ponce is licensed as a Skilled Nursing Facility, we are known for our excellent rehabilitation results. This is largely because our goal is to return residents to their home.

Nursing

This team works to promote a more hopeful attitude and helps to identify the goals of each resident and their families.

Activities

The Ponce offers a wide variety of activities seven days a week, including daily news, music, and arts & crafts.

Nutrition

Choices are offered for every meal, and our full time Registered Dietician and Certified Dietary Manager are well equipped to meet the needs of those with special dietary considerations.
