At The Forum at the Crossing we offer a full continuum of care, including Independent Living, Enhanced Services, and Memory Care, as well as Skilled Nursing and Rehabilitation.

We also welcome short- term stays in our apartments and health care center based on availability. Outpatient services, including physical therapy, occupational therapy, and speech/language therapy are provided by our on-site Rehabilitation and Wellness staff.

Enhanced Services

Enhanced Services packages are available for Independent Living residents who may require assistance with activities of daily living, such as medication set-up, bathing, or dressing. These services are tailored to the individual’s unique needs and preferences. Regular assessments and specialized care plans ensure the resident’s comfort and safety.

Our Memory Care neighborhood is specially designed for seniors with Alzheimer’s Disease or other cognitive impairment. Our unique care approach is called Bridge to Rediscovery, and it begins with knowing each resident as an individual, and learning his or her life story as well as individual care needs.  

Through Bridge to Rediscovery, we encourage our residents to participate in daily activities they can enjoy within a secure environment. Our specially trained staff provides personalized care and support 24 hours per day, and works throughout each day to help residents to continue to engage in the world around them.
