Sweetgrass Village Assisted Living Community offers a wide variety of services and amenities including:

• 24-hour specially trained on-site staff
• Three nutritious meals served daily

• All utilities paid except telephone and cable
• Apartment maintenance
• Emergency call systems in each apartment

Personal Assistance

Our goal at Sweetgrass Village Assisted Living Community is to meet each resident’s needs while maintaining the highest level of choice, dignity, privacy respect, and individuality. To assure that this is done effectively, an assessment is conducted prior to move-in. Residents, family members and Sweetgrass Village staff work together to develop an individually- tailored Assistance Plan.

A follow-up family care conference will be held regularly to assure that as a resident’s needs change, the Assistance Plan is adjusted accordingly. Our Personal Assistance Plans support our residents in the following areas:

• Assistance with medication management
• Medical appointment scheduling
• Assistance with personal care such as
• Modified diets or snacks bathing, dressing, and grooming
• Select diabetic care
• Health services: monitoring, supervision, or assistance
• Personal laundry service
• Safety and security checks
• Escort assistance to meals & activities
• Additional housekeeping services
• Mealtime reminders
• Periodic nighttime care
