Assisted Living and Memory Care

At Summit Place of SouthPark we provide Assisted Living and Memory Care for those who require some level of help with the Activities of Daily Living (ADLs) such as eating, bathing, dressing, laundry, housekeeping, medication management and more. These services are overseen by our Certified Nursing Assistants and Licensed Medication Technicians, whom are managed by our community Registered Nurse and Licensed Practical Nurse.

Upon admission, we create a Care Plan for each individual resident. Our Care Plan details the personalized services we will provide to the resident to improve their quality of life and to maintain a level of independence. The Care Plan is updated regularly to assure that the resident receives the appropriate level of care.
