
Through a wide range of services, our staff are prepared to meet the physical, emotional, and spiritual needs of residents. The beautiful 240-bed facility is located on the St. Vincent's HealthCare main campus in the Riverside area of Jacksonville, FL. As one of the largest long-term nursing facilities in Jacksonville, SCLM offers many of the services residents need in a home-like atmosphere. Services include a 60-bed Special Care Unit for Alzheimer's and Dementia residents.
SCLM is conveniently located on the picturesque St. John's River. The Semmes Garden, located along the river, is easily accessible and features gazebos, a fountain and landscaped walkways for residents and their guests to enjoy the beautiful rose garden and other foliage throughout the year.
Our professional staff includes a licensed Nursing Home Administrator who is certified and serves as a preceptor for administrative interns. The Assistant Administrator is an RN and is licensed and certified with a master's degree in Health Care Administration.
The staff also includes a Certified Director of Nursing, an Assistant Director of Nursing, licensed clinical social workers, discharge planners and a Certified Activities Director. The Medical Director is a Family Practice physician.
Therapy Services: SCLM contracts with outside rehabilitation providers for its Physical, Occupational and Speech Therapy Services. Upon admission for short-term or long-term care, a licensed and qualified therapist will assess the new resident and make recommendations to the attending physician. With physician orders, the therapies are provided until the resident meets his and his physician's goals, or until he will no longer benefit from the services. Throughout the resident's stay there are opportunities to meet with the interdisciplinary team providing care. This allows the resident and his family to get a "whole picture" of his rehabilitation and to better plan for his care needs after discharge.
Nursing Services: St. Catherine has a large staff of dedicated and caring nurses and CNA's that primarily provide for the daily needs of residents. Each resident neighborhood has a nurse clinician and a full staff of RN's and LPN's. All nursing assistants that are employed at St. Catherine are certified through the State of Florida.
Dietary Services: A licensed, registered Dietitian is available to consult with reisdents and families concerning speical dietary needs and requests. The Dietitian is available five days a week. We suggest that appointments be made with the Dietitian as dietary concerns arise. Residents are encouraged to eat in the dining areas to participate in socialization with other residents. However, in-room dining is available. Coffee is available in the main dining room and is complimentary for residents, staff and guests.
Laundry Services: Bed linens, towels, blankets, etc. are routinely laundered by the Facility Services Department. Residents' clothing and personal washable items may also be laundered by that department upon request.
Beauty and Barber Shop Services: SCLM Beauty Shop is located on the second floor, with hours varying depending on appointments but generally between 10 a.m. and 4 p.m. Monday through Friday. Customers are seen by appointment or walk-in, on first come first serve basis.
Social Work Services: SCLM associates understand the difficulty of adjusting to a new home and the special needs of the individual long-term care resident. Clinical Social Workers provide therapeutic counseling services for adjustment and conflict resolution issues that may occur during the initial adjustment period and at intervals thereafter. The social workers also provide the resident and family with assistance for transportation services, financial assistance programs and other community services that may help to address the psychosocial needs of the reisent/family.
Pastoral Care: The chaplains provide spiritual and pastoral care for our residents, their families and the staff. The chaplains are part of the interdisciplinary teams that meet weekly to oversee and review the residents' care on a timely and rotating basis. The chaplains are always in the process of creating an environment of trust and affection. Chaplains are typically available 8 a.m. to 4 p.m. Monday through Friday. The Pastoral Care Office is located on the first floor next to the chapel.
Activities: Opportunities for socialization, entertainment and therapeutic activities are offered daily with the goal to help every resident stay active for as long as possible.
