Since 1982, Southlake Nursing and Rehabilitation Center has been providing nursing and rehabilitation services to our community. Located in the beautiful residential community of Mandarin in Jacksonville, Florida. Southlake Nursing & Rehabilitation Center is a free-standing licensed 180-bed Medicare, Medicaid and VA certified facility. We specialize in short term skilled nursing and rehabilitation, long term, respite and end-of-life care. Our Safe Harbor Unit provides specialized services for those residents that are memory impaired and tend to wander. We cater to the geriatric population and those adults needing short-stay services.

Southlake Nursing & Rehabilitation Center provides short term nursing and rehabilitation as well as long term care in a family atmosphere where the well being of each resident is our number one priority. To fulfill this commitment to quality, we carefully review each resident prior to admission to assure that we can meet the needs of each prospective resident and the expectations of their loved ones. Our direct care givers are licensed, certified or registered according to the professional standards of the State of Florida.
Nursing Services
Our nurses specialize in 24 hour skilled nursing care for geriatric and qualified adults with special needs such as:

The Safe Harbor Secured Unit is designed for Alzheimer’s and Dementia residents to have a safe open space to wander while reducing the risk of outside dangers. The unit is staffed with compassionate professionals who have received special training in dementia disease, treatment and interventions.The large, home-like multipurpose dining and activity room allows for a variety of group and individual activities to occur at the same time. Off this room is a large screened porch where residents may enjoy the motion of rocking chairs or just sitting outside and enjoying the courtyard. The enclosed, spaciously landscaped courtyard is equipped with a security system, gazebo, water fountain and a walking path.
Our philosophy of care in the unit is to promote security, comfort, dignity and compassion to those afflicted with this disease. We also work closely with families of these residents by inviting them to join us with our Alzheimer’s Support Group for education and friendship.

Physical Therapists are licensed health care professionals who evaluate and treat people with health problems resulting from injury or disease. Our Registered Physical Therapists are seasoned professionals specialized in treating the geriatric population. Below are examples of injuries/diagnoses that our Physical Therapists treat on a daily basis.
- Orthopedic injuries including sprains, trains, broken bones, or muscle tears
- Neurologic injuries that interfere with a patient’s function and/or independence such as Parkinson’s disease, Multiple Sclerosis, Dementia or spinal cord injury
- Person’s suffering from chronic illness/disease, such as arthritis, COPD or fibromyalgia
- Person’s with an amputation of a limb
- Person’s with wounds including those with arterial and venous insufficiency
- Person’s suffering from pain
Occupational Therapy Services
Occupational Therapists are licensed health care professionals who evaluate and treat people with problems that affect their Activities of Daily Living (ADL’s). ADL’s include dressing, hygiene, bathing, and eating.They also focus on adaptive equipment and cognitive re-training to assist our residents attain their highest level of function. Our Registered Occupational Therapists have specialized in treating the geriatric population for over 25 years. Below are examples of skills re-training that our Occupational Therapists perform on a daily basis.
Speech Therapy Services
Our Speech Language Pathologists specializes in evaluation and treatment of Adult Neurological Disorders which may cause a person to have difficulty with communication and/or chewing and swallowing abilities. Our Speech Language Pathologists have specialized in treating the geriatric population for over 33 years. Treatment is provided for disorders such as: Aphasia, Voice Deficits, Speech Deficits, Cognitive-Linguistic Defects and Dysphasia (swallowing difficulties).
In most cases, patients are referred to a skilled nursing or long term care facility during hospitalization. The continuum of care process within the healthcare community can be overwhelming for those of us that are not familiar with it. We are here to help you with the process.When a patient no longer meets the medical requirements for hospitalization and connot safely return home, their discharge planner and physician refer the patient and/or legal representative to a skilled nursing or long term care facility for continued services.
Although there may be pressure to choose a skilled nursing or long term care facility quickly, you are strongly urged to tour the facilities under consideration before making a decision. During your visits try to speak to staff and family members of residents. Ask to meet the Administrator. He/She should be accessible and willing to speak with you.

Medicare Part A
Medicare is the national health insurance program for people age 65 or older, some people under age 65 with disabilities and for people with End-Stage Renal Disease (ESRD).To qualify for Medicare Part A Skilled Nursing Coverage, the beneficiary must spend at least 3 full days in a hospital, must have a skilled nursing need and have a physician’s order. The beneficiary is entitled to 20 days at 100% coverage and 80 days at 80% coverage per spell of illness. After day 20 the beneficiary will be responsible for the 20% co-pay, unless he or she has supplemental coverage.
There is a misconception that Medicare Part A covers up to 100 days regardless of need.
Institutional Care Program (ICP) Medicaid
Medicaid is a welfare program that pays for medical assistance for certain individuals with low incomes and resources. This program became law in 1965 and is jointly funded by the Federal and State governments to assist States in providing medical long-term care assistance to people who meet certain eligibility criteria. Medicaid is the largest source of funding for medical and health-related services for people with limited income.
Making an Informed Decision
Skilled nursing and long term care facilities vary in the level of overall health and functional impairment of individuals and in admission and discharge practices. These differences can affect scores on the quality measures but do not reflect the quality of care provided by the nursing homes. Unless scores are adjusted for such factors, a quality measure may not give a true and fair picture of clinical care being provided.
