For patients who require a higher level of care than can be provided in their home, but do not require hospitalization, Sharp's skilled nursing facilities can be the answer to a wide range of needs.

Rehabilitation

A wide variety of rehabilitation therapies are available to patients with impaired functional abilities due to stroke, brain injury, spinal cord injury or other neurological conditions. Those with disabling conditions, including amputation, severe arthritis and those caused by orthopedic (also spelled orthopaedic) or cardiac conditions also have access to our highly qualified physical, speech and occupational therapists.

 

Recuperation

Many patients who have undergone surgery may no longer require acute hospital care but are not yet ready to return home. They are medically stable, but require more services than can be provided through home health care. A skilled nursing facility can provide these individuals with the specialized recuperative care they need and improve their transition from surgery to home. This level of care is appropriate for patients recovering from procedures such as hip replacement, tracheostomy or other post-surgical needs.

As today's health care insurers are forcing shorter hospital stays, an interim setting is often required upon discharge from the acute-care hospital. To assist our patients' successful transition from the acute hospital setting to home, or another more appropriate level of care, Sharp HealthCare provides specialized nursing units to bridge the gap between hospital and community re-entry. These transitional care units are designed for those patients requiring more intensive nursing and medical care that can be provided in a long-term care facility.

Our subacute nursing units offer comprehensive medical care that serves patients recuperating from surgery, strokes, broken limbs, persistent sores and other conditions once treated solely in hospitals. Patients eventually go home, though some do transition to other, more appropriate levels of care, as determined by their personal physician.
