Our Geriatric Care Management team of licensed nurses, licensed social workers, and certified caregivers provides high quality care at home that improves your medical, functional and emotional well-being.
Our company’s unique approach addresses the total well-being of clients and their families through a comprehensive program that includes assessment, planning, service coordination, advocacy, and direct care by a interdisciplinary team led by a geriatric care manager.
Our professionals include highly trained, experienced and licensed social workers, nurses, exercise and nutrition experts, and in-home caregivers. This highly skilled team coordinates care with the client’s physician(s) so that medical problems are fully assessed and addressed. The full range of services is personalized according to the level of need and preference. The SeniorBridge program is especially beneficial for people with complex problems, chronic medical illnesses or memory disorders such as Alzheimer’s disease; those who require close oversight and coordination of care.
Without the type of care offered by SeniorBridge, people with chronic conditions would be placed in nursing homes, dementia units or other type of long-term care institutions. SeniorBridge provides the needed care outside of such institutions, thereby enabling people to live safely in the comfort of their own homes.
SeniorBridge care is based on an integrated practice model. By coordinating the multiple disciplines needed to ensure the proper medical, social, functional and support needs of the client, SeniorBridge provides the highest quality of care. Our goal is to ensure the best quality of life for each of our patients.
When appropriate, a companion or home health aide is selected to provide direct care services for the individual. Companions and home health aides are available on an hourly or live-in schedule. Our staff is available on a full-time or as-needed basis 24 hours a day, 7 days a week. Our in-home care givers are trained and supervised by our nurse and social worker care managers. There are a number of tasks and activities our care giving staff can provide, including, but not limited to, the following:

Assistance with daily activities such as grooming and dressing, cooking and eating, bathing, toileting and moving
- Help with activities such as shopping, driving, housework, laundry, using the phone, handling money and taking medications
- Socialization and companionship
- Facilitating special interests such as the arts, languages, and sporting activities
- Assistance while traveling
