Rocky Mountain Assisted Living
Rocky Mountain Assisted Living is dedicated to providing you with all the care and resourcs you need to live in a home-like setting.
Rocky Mountain Assisted Living is owned and operated by health care professionals with over 10 years of experience in the senior care industry. We strive to provide the best possible care for our residents by staying involved with our employees and residents on a daily basis.
We carefully supervise and monitor all of our residents care. We encourage open communication from the family and residents.
Each home has a house manager and care staff team that is trained in proper care techniques on an ongoing basis. In addition they receive support and information from a Colorado State licensed Administrator. Each team member is empowered to do whatever they can to make each person feel like they are still in their very own home. From favorite family meals to familiar activates.
We are not managed or owned by a large out of state Corporation or Management Company. We feel that in order to really meet the needs of our residents and their families we need to be on the ground level.
Independent Living
Independent living is an apartment like setting in an age restricted community. There are social activities and services available there like 1-2 meals per day, housekeeping services and transportations. There are typically no personal type care services available for you in independent living.
Assisted Living
Assisted living residences or assisted living facilities (ALFs) provide supervision or assistance with activities of daily living (ADLs); coordination of services by outside health care providers; and monitoring of resident activities to help to ensure their health, safety, and well-being. Assisted Living is an intermediate step between living independently and living in a nursing home.
Secured Assisted Living or Memory Care
Alzheimer special care units (SCUs) (also known as “memory care” assisted living) have been developed to meet the unique needs of people with Alzheimer’s disease and other dementias. SCUs can take many forms but most often exist as cluster settings in which people with dementia are grouped together on a floor or a unit within a larger residential care facility. SCUs offer a higher ratio of staff to residents than other types of care, specialized training for their staff, and activity-based patient programs. The unit may include features such as secured exits and enhanced visual cues (such as signs or pictures) designed to help residents feel more oriented in their surroundings.
Skilled Nursing Facilities/Rehabilitation Centers
Skilled Nursing Centers provides a place of residence for people who require constant nursing care and have significant deficiencies with activities of daily living. Residents include the elderly and younger adults with physical or mental disabilities. Residents in a skilled nursing facility may also receive physical, occupational, and other rehabilitative therapies following an accident or illness.
Continuing Care Retirement Community (CCRC)
Also known as a life-care community, this is a type of retirement community where a number of aging care needs, from assisted living, independent living and nursing home care, may all be met in a single residence, whether apartment or enclosed unit. Typically, elderly candidates move into a continuing-care retirement community (CCRC) while still living independently, with few health risks or healthcare needs, and will reside there until end of life. As residents progress in age, and medical needs change, the level of nursing care and service increases proportionally in response. In such a way, the needs of patrons are consistently monitored and catered to, particularly as those needs become more intensive.
In-Home Care
In home care refers to custodial or companion care in a person’s own residence. This is not to be confused with Home Health Care. This service allows individuals to remain at home with one on one care assisting them with the activities of daily living (ADL’s). This service is not typically medical in nature. This service is not covered by Medicare. Most individuals pay for this out of pocket, long term care insurance, or by Medicaid in some states.
Home Health Care
Home health care is for short term episodes after surgery or a hospitalization. These services provide the following types of care providers, physical therapy, occupational therapy, speech therapy, skilled nursing, social worker and certified nursing aide. This service is paid for by Medicare and commercial health insurance.
Adult Day Care
This service is also commonly known as adult day services. It is a non-residential facility providing activities for elderly and/or handicapped individuals. Most centers operate 10 – 12 hours per day and provide meals, social/recreational outings, and general supervision. Adult daycare centers operate under a social model and/or a health care model. These services are paid for by private pay, Medicaid and long term care insurance.
Hospice Care
Today hospice care provides humane and compassionate care for people in the last phases of incurable disease so that they may live as fully and comfortably as possible. Hospice care is for individuals who have been diagnosed with 6 months or less to live. Hospice care is covered by most health insurance plans. Hospice care involves treatment of pain and is provided by a registered nurse, chaplain, social worker and nursing aide.
