Being surrounded by family, friends and the things that make a house a home is the very essence of home care. We will provide services in the comfort of your home while you’re recovering from an illness or surgery. Skilled Nursing, Physical Therapy, Occupational Therapy and Speech Therapy are on an intermittent basis in accordance with your physician’s orders. We strive to support healing at home and our primary purpose is to educate patients and their caregivers. This education will enable patients to promote self-care and remain in the comfort and independence of their own homes for as long as possible.

Our skilled professional staff is trained to provide comprehensive care to address your specific needs. Our multi-disciplinary team includes skilled nursing, enterstomal/wound care, diabetic teaching, psychiatric nursing, physical therapy, occupational therapy, speech language pathologist, registered dietitian, medical social worker and home health aides.*
Home health nurses may help you with:

Diabetes – Teach diabetic management, self care; teach glucose monitoring - Cardiovascular Disorders – Educate on signs and symptoms; assess cardiopulmonary status
- Surgery – Post-op management
- Neurologic Care – Strokes, post-op neurosurgical care and other neurological deficits
- Medication/Diet Changes – Teach medication and adherence with new/old regimens
- Asthma/COPD – Educate on disease management
- Wound Care – Assess for signs of infection and dressing changes
What does a physical therapist do?
- Exercises for strengthening
- Walker, cane and assistive device teaching
- Teaches gait training
- Uses ice packs and hot packs
- Traction in your own home
- Total joint rehabilitation
