Pax Hospice

About Us

Pax Hospice is dedicated to providing compassionate services to persons with a Terminal illness and to their families through education and emotional support. We recognize the patient as an individual with physical, social, spiritual, and emotional needs.We believe that the patient is entitled to be as free of pain and symptoms as possible and to live his or her remaining days in comfort. The patient and family are entitled to decide what care they wish to receive and to be treated with respect and dignity.

Care Givers

Medical Director

Responsible for direction of the care and treatment to hospice patients rendered by the hospice care team. Assess patients' medical needs.

Registered Nurse

A member of the Interdisciplinary Team and works under the direction of the Attending Physician and the Medical Director in the provision of skilled nursing assessment, planning and care according to the Interdisciplinary Plan of Care in order to maximize the comfort and health of the patients and families.

Hospice Aide

Provides direct patient care and/or house keeping duties in the home or homelike setting under the direct supervision of a registered nurse.

Social Workers

Will assist the Physician and other IDT members in understanding significant social and emotional factors related to the patients health status. 

Chaplains

Provides spiritual counsel and emotional support to the active and bereaved patient/family units and liaison between community clergy and Hospice. The Hospice Chaplain is committed to the philosophy, goals and objectives of the Hospice concept.

Volunteer Coordinator

Responsible for recruiting, orientation, training, coordination and retention of all Pax Hospice volunteers.
