Parkshore Senior Community is one of the premier senior housing communities in Minneapolis. Located in the heart of St. Louis Park adjacent to Wolf Lake Park, our independent-living, assisted-living, and memory care apartments are located in a “walk around” neighborhood with area amenities that include  Byerly's, Target, and Park Nicollet Clinic.

Our independent and assisted living buildings are connected to provide easy access to a variety of services and programs.

Assisted Living

For those who desire personal care and support, Parkwood Shores is the wise choice for assisted living. Individual apartments offer privacy and the feeling of home. Studio, one and two-bedroom apartments include a kitchen, private bath and dining/living rooms. Close at hand are lovely common areas for socializing and meeting new friends. Meal times are treated as special occasions with tableside service, restaurant-style menus and entree options. The dining room is decorated to provide a warm, friendly atmosphere that is sure to please residents and family alike.

Our home care provider, AgeWell, designs personalized care plans encouraging independence and offers the support needed to live in a private apartment home. An array of daily scheduled activities offer many opportunities to stay active and involved.

AgeWell – Life Care Management

Personalized Care services are developed by AgeWell, your home healthcare provider to ensure a unique, holistic model of care that goes beyond just health services.

• Each resident is assigned a Life Care Manager (a licensed nurse), who will address cares and concerns, while providing a proactive, preventative approach to well-being.
• AgeWell Life Care Managers (licensed nurses) are on site 7 days a week, and available by phone 24/7. Their passion is making each day special for our residents.

Integrated Medical Rehabilitation

IMR Provides physical, speech, occupational, and respiratory therapy. Residents can be seen by a therapist without having to leave the campus. IMR accepts all commercial insurance and Medicare Part A. IMR partners with AgeWell’s Life Care managers and the resident’s personal physician to ensure a coordinated approach to care.
