We provide superior care and customer service for Skilled Nursing, Wound Care, Physical Therapy, Occupational Therapy, Speech Therapy, Home Health Aides, Medical Social Workers. With many offices offering in home Psychiatric Nursing, IV Therapy and Chemotherapy.

OMNI Home Care was founded in 2001 and has since grown to more than 1000 employees, with service areas throughout Florida, Pennsylvania, Illinois, Indiana and Ohio. We are continuing to grow and expand to other areas in the United States. Our corporate headquarters is located in Coral Springs, Florida.
OMNI Home Care recently merged with SunCrest Healthcare, Inc. SunCrest was formed in November 2005 for the purpose of developing Medicare/Medicaid-certified home health agencies in the Southern United States. The company is based in Nashville, Tennessee. SunCrest’s operating philosophy focuses on developing effective partnerships with the local healthcare communities, especially the local hospital systems, in order to ensure that the local communities’ home health care needs are met in a caring, high quality, safe, efficient and cost-effective manner. SunCrest operates locations across Tennessee, Alabama, Georgia, Florida, Kentucky, Mississippi and Missouri.

Senior Executive Team
Fred Portnoy, Chief Executive Officer
Fred joined OMNI during 2004 as the Executive Vice President of Operations. He was promoted to President in December 2007 and then promoted to CEO in May 2008.Prior to joining OMNI, Fred was the Chief Operating Officer (2001 to 2003) of Vivra Asthma & Allergy, a $50 million national physician practice management company, where he successfully restructured the operations and then sold off all of the operating assets for two private equity firms.
Tracy Clark, Executive Vice President - Corporate Development
Tracy joined OMNI in 2002 as the Operating Officer, responsible for all operations and development in OMNI's initial start up stages, and is now Executive Vice President of Corporate Development. In the initial stages, he directed OMNI‘s initial startup operations in 2002 to 20 locations by 2005 and annual growth of over 50%. Currently he leads OMNI’s Employee Relations, Startup Development and Sales Strategy. Tracy has been in business development and sales leadership roles in healthcare for over 20 years in homecare, physical/occupational therapy, physician practice management and outpatient surgery centers.
Jenice Carrick, Chief Compliance Officer
Jenice Carrick joined OMNI as Chief Compliance Officer in May 2010. Prior to joining OMNI, Ms. Carrick was the Area Vice President of Operations for Amedisys, Inc., overseeing regional multi-site locations in Florida as well as the organization’s first international venture in Puerto Rico. Her primary responsibilities with this publically traded industry leader included fiscal management, clinical operations and business growth strategies of assigned territory. She successfully launched multiple start-up locations and assimilated competition through mergers and acquisitions.
Steve Marble, Chief Financial Officer
Steve is a Certified Public Accountant and joined OMNI in late 2010 as Chief Financial Officer.Prior to joining OMNI, he worked with Monomoy Capital Partners as CFO (2009-2010) at two portfolio companies where he helped implement various turnaround and restructuring activities.
John Kiehl, Vice President of Clinical Support Services
John joined OMNI on December 1, 2007 and is primarily responsible for all information technology as well as leading several large projects that focus on enhancing operational efficiency. John works directly with each branch, focusing on key objectives, and supporting and assisting in achieving operational excellence.

OMNI Home Care provides a variety of services:
Skilled Nursing - assesses condition and teaches the patient and/or their family the signs, symptoms, complications, medications and side effects as well as nutrition.
Physical Therapy - rehabilitates the patient to their fullest potential in their own environment, teaching them to use medically necessary devices.
Occupational Therapy - teaches the patient to perform activities of daily living in the environment they will need to work in.
Speech Therapy - teaches communication skills, exercises swallow and gag reflexes in privacy.
Medical Social Worker - assists the patient/caregiver, physician and agency in short and long-term planning, linkage with community resources, and assess social and emotional factors that may impact care.
Home Health Aide - may assist the patient in their activities of daily living for a short period of time while they rehabilitate and become more independent.
Specialized Care Programs - OMNI Home Care provides Care Management Kits when ordered by the physician, as part of the plan of treatment to improve the patient 's overall outcome and quality of life. Our highly skilled and trained health care staff provides counseling and education in the home as part of the overall plan of care. We offer a wide variety of programs that address multiple disease states. OMNI 's kits include instructional booklets, logs, and other materials depending on the program.
