Our tailored rehab plans are designed to assist each patient to regain his or her strength and independence.  Our post-hospitalization care provides:

At the time of discharge, our interdisciplinary team will coordinate the necessary services to ensure a smooth transition. We will provide family education to help the family assist a when they return home.

Our comprehensive plan of care encourages resident as well as family involvement. Families are invited to attend interdisciplinary meetings during their loved one’s stay.
