Nazareth Home
A long-term care facility with licensed nursing facility beds, and licensed personal care beds. Nazareth Home provides three levels of care: Personal, Intermediate, and Skilled.
Short term orthopedic rehabilitation and recovery to home program: Nazareth Home’s 17-room recovery to home program is designed for those who need short-term recovery following a hospitalization or a surgical procedure such as joint replacement. This new specialty unit was created in 2006 at the request of the community due to shorter hospital stays and the need for specialized recovery and daily rehabilitation.
Since 1976, Nazareth Home has played an active role in serving Louisville’s elder and disabled population. When the community needed an innovative program to serve adults with active HIV disease, the Sisters made Nazareth Home available to the AIDS community for nursing home care. While the needs of persons living with HIV/AIDS no longer made special accommodations necessary, this history is an example of the type of leadership in long term care demonstrated by Nazareth Home.
In May, 2007, Nazareth Home added a 17-room memory care neighborhood for those persons in the early stage of dementia. The Memory Care Center was initiated at the request of families of current and past residents who saw the need for a personal care program that had both independence and security. Our Charity Court program serves 31 persons whose dementia presents a need for the nursing home level of care. The goals of each program are to build on strengths and desires, both past and present, of families and residents creating a supportive community for residents and their families. Our person-centered philosophy of living creates a defined professional staff living and working together with residents and families to foster a high level of trust and satisfaction.
