About Us
What began as a small home health care service in 1978 has evolved into Mid-Delta Home Health and Hospice. Mid-Delta Home Health and Hospice is a full-service health care provider offering a broad spectrum of services and programs. These services and programs are tailored to patients who require health care services in the home. From our hearts to yours, Mid-Delta Home Health and Hospice strives to be the premier health care provider. We have distinguished ourselves in the marketplace by offering quality focused, cost-effective, comprehensive health care services and programs.
Mid-Delta is a unique provider in that we believe in the holistic approach to patient/client services. Mid-Delta's holistic approach addresses the well being of each of our patients regarding their physical, mental, social, and spiritual needs. Individuals can opt from one service to another without changing nurses/aides or therapists. Example, a patient may transfer from Home Health to Hospice or vice versa. Working with each patient's personal physician, your Mid-Delta patient care team develops an individual plan of care designed to meet the needs of each patient and their families. In addition, we work with local communities to seek out sources of financial aid and assistance. Our multi-disciplinary team of health care professionals are comprised of highly skilled---Physicians, Registered Nurses, Licensed Practical Nurses, Certified Nurse Assistants, Home Health Aides, Sitters, Licensed Social Workers, (full time) Registered Dietitians, Occupational Therapists, Physical Therapists, Speech Therapists, (full time) Respiratory Therapists, and Clergy.
Home Health Care
The benefits of home care are:
- Early patient discharge from the hospital
- Patients remain more independent and enjoy greater comfort and convenience
- Patients and families are more involved in the treatment and healing process
- Patients favor the safety and familiarity of their own homes-and heal quicker
Hospice Care
Hospice services provide special care for terminally ill patients who can no longer benefit from curative care. The patient remains with their family at home in familiar surroundings while receiving care from a specially trained hospice team.
Home Infusion
Home infusion specializes in providing the highest quality pharmaceuticals and related services in the home. Our pharmacists and supportive staff have extensive experience in home IV infusion therapies with both adult and pediatric patients. We utilize state-of-the-art equipment in preparation and delivery of IV infusion therapy. We work closely with the physician to monitor and evaluate the patient’s response to our patient’s individual plan of care.
Pediatric Services
From infant to adolescents, Mid-Delta provides a wide range of homecare and educational services specially designed for children. We believe that promoting the integrity of the family unit enhances the quality of life for the patient. To achieve this goal our skilled professionals encourage family involvement in all aspects of planning and caring for the child. Furthermore, our in-home care is designed to assist and support parents and/or caregivers to obtain the knowledge and skills that will enable them to independently provide safe, knowledgeable, and competent care to their children.
Psychiatric Services
Mid-Delta Psychiatric Services uses a multi-disciplinary team approach to psychiatric home care. Our specialized nurses, medical social workers, and other team members, work closely together to meet the individual needs of each patient. The goal of psychiatric home care is to help the patient maintain mental stability attained while hospitalized and to prevent re-hospitalization. Patients who experience situational short-term emotional problems due to illness, surgery or life changes may also benefit from psychiatric home care services. Our staff provides comprehensive psychiatric assessments to determine intervention for psychiatric diseases.
