Our wide array of services enables our patients and residents to receive the medical care they need, the restorative therapy they require, and the support they and their families deserve.
Our nursing and rehabilitation center offers short-term rehabilitative as well as long-term care - a full range of medical, nursing, rehabilitation and social services to treat and support each of our patients and residents. Many of our patients go home after a few weeks and, for those who are unable to return home right away, we provide safe, compassionate care in an environment that fosters independence and dignity.
We serve many types of patient and resident needs – from short-term rehabilitation to traditional long-term care.
Working with your physician, our staff – including medical specialists, nurses, nutritionists, therapists, dietitians and social workers – establishes a comprehensive treatment plan intended to restore you or your loved one to the fullest practicable potential.
Our center takes care of people who have complex medical conditions that require brief or prolonged treatment. Typically, these patients have medical conditions that require the expertise of an interdisciplinary team of healthcare professionals. The team includes the patient or resident’s attending physician, medical specialists, therapists, registered and licensed nurses, social workers, registered dietitians, activity directors and nursing assistants.
As part of a nationwide Kindred Healthcare network of skilled nursing facilities and long-term acute care hospitals, we benefit from clinical systems that help us provide continued enhancements in quality.
Upon admission to our center, all patients and residents are assessed by members of our interdisciplinary team to determine what care is required and a comprehensive, customized treatment plan is developed. Patients are assisted to reach their goals for recovery in a home-like environment by our team of caring and highly-qualified healthcare professionals, helping each patient and resident reach his or her highest level of functioning.
When patients leave the hospital, they often need continued care to recover completely. That’s where we come in. We work with you to set a care plan to meet your goals and abilities. Patients typically receive up to three hours of intensive rehabilitation each day, provided by our specially trained interdisciplinary team consisting of:
- consulting physician specialists
- nurses
- rehabilitation therapists
- respiratory and speech therapists
- social workers
- registered dietitians
Our team treats a variety of conditions including:
- post-stroke recovery
- hip fracture/replacement
- knee or other joint replacement
- neurological illnesses (Parkinson’s disease, etc.)
- cardiac or respiratory ailments
- tracheotomy weaning
- post acute COPD episode
- post ventilator care
- post open heart (grafts, valves, stints)
Long-term care is when a person requires assistance with his or her physical or emotional needs over an extended period of time. The need might be due to a terminal condition, disability, illness, injury or the infirmity of old age. The need for long-term care may only last for a few weeks or months, or it may go on for years.
We provide the services and supplies that your family member needs while he or she is a resident at our center. This help may be required for many of the activities or needs of daily living, including:
