About Us
Leon Sullivan Health Care Center Named in honor of the Rev. Dr. Leon H. Sullivan, humanitarian and recipient of the Presidential Medal of Freedom; founder of O.I.C. of America and O.I.C. International.
Life experiences are different in every culture. We provide individualized care that addresses the ethnic and cultural needs and beliefs of our residents through sensitive meal planning, religious activities, and holiday observances in conjunction with other daily activities that give residents a feeling of continued connection with their life and family experiences.

We envision becoming a leader and preferred provider of high quality skilled nursing care and sub-acute rehabilitation services in King County. Our Mission is to provide medical and therapeutic support to people in need, to assist them in living their lives with dignity, high self-esteem, comfort and peace. To do this, we provide advanced health care techniques and compassionate care by trained health care professionals, within an aesthetically superior facility.
Comprehensive Rehabilitation program
Our professionally licensed therapists evaluate and establish treatment programs that assist and encourage each resident to reach the maximum level of independence possible. We aim to improve strength, activity tolerance, flexibility, balance, safety with functional mobility, moving in their environmen, and pain reduction.
Occupational Therapy
Our Occupational Therapy assists residents in regaining skills of everyday activities and to assess for less restrictive adaptive device for ambulation To assist clients in regaining the strength, mobility, function, cognition, safety awareness needed to complete activities of everyday living including Community mobility evaluation and proper fitting of wheelchairs and other medical devices to assist client in exploring their environment as independently and safely as possible.

Speech and Language Therapy
To help residents with speech, language, swallowing, and cognitive retraining.
Focusing on:
- Listening: audiotory comprehension
- Swallowing: Vital Stim certified
- Thinking: attention immediate memory, verbal thought orgamization, executive functioning and planning
- Speaking – Voice LVST certified – Lee Silverman voice program
Other Services
Social services, mental health services, volunteers and recreational staff are available to provide supportive services during all stages of adjustment. Nursing services are administered 24-7 by a team of professionally trained and certified nurses and other caregivers experienced in meeting the healthcare needs of our residents.
