About Us
Family is the core value of Legacy Care Home and our family stories created the foundation for Legacy Care Home.

Bev and Jim Carlson married, settled in Minnetonka, and raised two children. Family was very important to them. Bev was diagnosed with Multiple Systems Atrophy in 2000, which is a form of Parkinson's disease. It debilitated Bev rapidly. Bev's family pulled together to care for her at home. Jim, her primary care giver, realized how hard it was to care for Bev and hold a full time job. So the family used a residential care home on weekends for respite and pull together during the week to meet her needs. After three years of fighting the disease, Bev passed peacefully on October 15th 2003, at the age of 56.
Grandpa and Grandma Ranta married, settled in Plymouth and raised four children who have all settled locally to raise their own families. Grandpa struggled with Alzheimer's disease for three years. Grandma and the children cared for his needs at home as long as they could before his needs demanded more. Grandpa was cared for in a memory care facility for three years before his passing December 2005.
Grandpa and Grandma Paulson met, married, farmed and raised eight children in Minnetonka. Grandpa always kept his family close, wanting each one to take a piece of the family farmland. Grandpa died May 1997 leaving grandma to keep the family together. Unfortunately grandma suffered with Alzheimer's disease for four years before she passed in October 2007.
With all that, we decided to purchase a piece of family farmland in Minnetonka and begin Legacy Care Home. Legacy is our way of paying tribute to our loved ones who have endured debilitating illnesses such as Parkinson's which Lisa's mom faced and Alzheimer's which two of Michelle's grandparents faced. Michelle and Lisa both know how hard it is facing the challenges in caring for loved ones and facing the health care needs they may have. Legacy is in dedication of our beloved mother, grandparents, and family.
Health Care Services
The following are examples of services we provide under our Class F home care license.
- 24 hour on site resident assistant staffing
- Personal Cares
- Daily dressing and grooming
- Bathing one time per week
- Controlled incontinence care
- Nail care
- Daily cueing and redirection as needed
- Medication administration
- Response by resident assistant to unanticipated needs
- Registered Nurse to coordinate care:
- onsite at various hours
- oncall 24 hours
- laboratory and Physician services
- Service Plan review every 62 days
- with other health care professionals
- Additional Services available from Legacy's home care agency as follows:
- Additional Skilled RN services
- Tube feedings, IV antibiotics, wound care, diabetic nail care, etc.
- Additional shower
- Unmanaged incontinence cares
- Resource and Referral Case Management
Housing Services
- Private rooms in our newly constructed dwelling.
- Occupancy and quiet enjoyment of the Unit, which includes wall-to-wall carpeting, window blinds, and bathroom.
- Utilities (heat, central air conditioning, electricity, water, sewage, garbage and waste removal, basic cable television).
- Three (3) meals per day.
- Housekeeping and laundry services equal to 1 hour per week for light cleaning of bedroom, bathroom and vacuuming of traffic areas of Unit and personal laundry and linens.
- Outlets for telephone.
- Scheduled local van transportation as may be provided for all tenants of Legacy Care Home from time to time.
- Recreational/health related/religious activities: as may be provided for all tenants of Legacy Care Home.
- Access to community and social events: as may be provided for all tenants of Legacy Care Home from time to time.
- Access to and use of all Legacy Care Home common areas, including kitchen, dining room, living room, porch, and decks.
- Storage unit space.
