Welcome to Kindred Transitional Care and Rehabilitation - Village Square On behalf of our entire team, thank you for considering our nursing and rehabilitation center for you or your family member’s care.
Our wide array of services enables our patients and residents to receive the medical care they need, the restorative therapy they require, and the support they and their families deserve. We serve many types of patient and resident needs – from short-term rehabilitation to traditional long-term care.
Working with your physician, our staff – including medical specialists, nurses, nutritionists, therapists, dietitians and social workers – establishes a comprehensive treatment plan intended to restore you or your loved one to the fullest practicable potential.

Specialty Programs
Our center is dedicated to being the provider of choice for health and rehabilitation care. We would like to introduce you to our recovery programs by describing key components. These programs were developed to meet the needs of the community and provide enhanced services for patients with specific health needs or chronic illness.

Orthopedic Program
Our center’s orthopedic program provides care for patients with muscle, bone and joint conditions including hip/knee fractures, lower extremity amputations, degenerative joint disease and musculoskeletal disorders.
Our collaborative interdisciplinary team approach to patient care is led by nursing and utilizes therapists from RehabCare, Kindred’s rehabilitation services division.
Our professional staff is trained in the latest treatment for orthopedic patients. Our program focuses on getting the patient back to their highest level of functioning and helping secure a smooth transition to home.
Pulmonary Program
Our pulmonary program is designed to assist our residents who have lung complications or disorders.
More than 20 years ago, Kindred Healthcare pioneered the first hospitals in the nation to specialize in pulmonary treatment, primarily ventilator management and weaning.
The pulmonary program available in our center shares that same tradition of quality care.
Wound Care Program
Wounds can occur for a variety of reasons, from post-surgery complications to normal skin aging.
Our center provides quality wound care designed to achieve positive outcomes and to help each patient and resident reach his or her highest level of functioning. Program goals focus on wound healing and prevention of skin care complications.
Our interdisciplinary wound care teams include physical therapists, registered dietitians and registered and licensed nurses with specialized training in treating wounds.
Types of Care
Short-Term Care
When patients leave the hospital, they often need continued care to recover completely. That’s where we come in. We work with you to set a care plan to meet your goals and abilities.
Cardiac Care
We provide cardiac care for conditions such as congestive heart failure high, blood pressure and post pacemaker insertion. Our interdisciplinary teams include physical therapists, registered dieticians, pharmacists, physicians and registered licensed nurses with specialized training in treating cardiac conditions.
Stroke Recovery Care
Stroke recovery provides treatment to help patients with strokes return to meaningful roles in life by relearning skills for every day living. This involves an Interdisciplinary team, including physical therapy, occupational therapy, speech therapy, nursing, pharmacy and physician services.
Respite Care
Our Respite Care Program includes:
- an individualized daily exercise program, social and recreational activities
- physical, occupational and speech-language therapies, as needed
- balanced, therapeutic meals, snacks and diet education
- activities that include arts and crafts, live entertainment and day trips
- religious services
Traditional Long-Term Care
Long-term care is when a person requires assistance with his or her physical or emotional needs over an extended period of time. The need might be due to a terminal condition, disability, illness, injury or the infirmity of old age. The need for long-term care may only last for a few weeks or months, or it may go on for years.
Dementia Care
The individuals we serve include those experiencing gradual memory decline or other memory, behavior and medical complications.
Palliative Care
Palliative care is any form of medical care or treatment that is designed to prevent and relieve pain and suffering, and to improve quality of life for people facing serious, complex illnesses. This can include psychological and spiritual care and developing a support system to help the individual live as actively as possible. Our team of caregivers, which often includes a social worker or a counselor, can play a role in helping the patient and family cope with the symptoms of disease and illness in ways other than medical/pharmacological intervention.
Hospice Care
"Hospice" comes from the Latin word for hospitality, referring to the warmth and courtesy extended to special guests, such as travelers who had fallen sick or otherwise needed a place to stay. Guided by a philosophy of compassion and dignity, our hospice approach centers on a dedicated team of healthcare professionals and volunteers who provide ongoing support for both patient and family in familiar surroundings.
