Sometimes taking medications can be very overwhelming and confusing to senior patients, especially if they have ongoing challenges with memory loss or declining mental status. We can definitely help you in this area as we have extensive experience in managing and organizing medications. We can eliminate duplicate medication bottles, provide color coding and medication charts as well as implement the use of a CMD (Computerized Medication Dispenser). In addition, we can fill the CMD or the patient's current pill box system with all their medicines on a weekly or biweekly basis. We would then order all necessary prescription refills in a timely manner from their pharmacy to ensure that they do not run out of their medications. With the CMD all stress and confusion is removed for your parent because it is a locked device that does everything for them except swallow the pills. This provides you with peace of mind knowing that they are taking their medications daily without any errors or problems.

We provide “TeleCare” which is a phone outreach service for patients who need a little checking on to make sure everything is going smoothly with them. Specially trained staff call the patient on designated days and asks 10 key questions about how the patient is getting along for that day. The patient's answers to those questions determine if there are any "yellow or red flags" present that could be a potential harm or negative consequence to them. The "yellow or red flag" information is then given to a geriatric care manager to determine whether an assessment call is needed for more information or if a patient visit is warranted for that day. The designated family member is then notified.

By being proactive with your parent or loved one, significant or potentially hazardous situations can be avoided or eliminated resulting in decreased ER visits, hospitializations, or crisis events. Proactive interventions are less costly, less disruptive, easier, and much more comfortable for your parent than reactive interventions. This service is especially helpful for patients with cognitive deficits, memory loss, or beginning stages of Alzheimer's Disease. This is an excellent stand alone service or can be added on as a "between visits" service to promote independence and keep that loved one at home. This phone service is available for as little as 1 day a week to as much as 7 days a week.

The Emergency Help System or “EHS” is a personal emergency response system with 2-way voice communication that makes live help just the push of a button away. Installation which we provide will connect your parent or loved one to a "live person" at the emergency response center by pushing a single “ help” button on the main transmitter unit. In addition there is the remote “help” button that is carried with them on their belt, pocket, on a chain around their neck, or on their wrist for that 24/7 availability of help. This life saving unit is rented monthly and is hooked up to the patient's telephone system for continuous peace of mind service.
