At Hospice Compassus, our mission is serving terminally ill patients and their families with compassion, integrity and excellence. Hospice Compassus offers the highest clinical standards for day-to-day medical care, including providing pain and symptom management. Our family-centered approach focuses on the patient, led by an interdisciplinary team, including a medical director, nurses, social workers, non-denominational chaplains, counselors and trained volunteers. Our goal is to provide the greatest possible comfort and care for those who experience one of life’s most intimate and challenging moments.
Our Care Team
At Hospice Compassus, care is provided by an interdisciplinary team of health care professionals who are skilled at serving people with life-limiting illnesses. Our team member’s work together to meet the medical, emotional and spiritual needs of patients and their families. The hospice team develops a care plan that meets each person's individual needs for pain management and symptom control.
Medical Director/Physicians
The Medical Director has overall medical responsibility for your care, working with your physician and other medical professions to develop a care plan that meets your specific needs. The Medical Director oversees the entire team, as well as the protocol for pain and symptom management. The medical director also makes house calls when necessary.
Nurses
Our nursing team is probably the most important patient advocate and most accessible member of the care team. A charge nurse will be responsible for your overall care and managing your needs. Nurses visit the patient as often as necessary and make arrangements for medication and equipment. They also provide support and valuable information to family members on how to care for their loved one.
Nurse Assistants & Home Health Aides
Nurse assistants and home health aides assist with day-to-day personal care, such as bathing and grooming. With regular visits, they also become a source of comfort, emotional support and companionship for patients and their families.
Social Workers
Our team of social workers lends support with non-medical concerns, such as legal and financial issues, access to community resources and future planning. They also address social and psychological needs by providing comfort and support to patients and family members who are dealing with stressful issues related to a life-threatening illness.
Chaplains
Our nondenominational chaplains offer spiritual support directly or in collaboration with the patient and family's clergy or spiritual counselor. Our chaplains honor the diversity in religious beliefs and backgrounds of our patients.xz
Bereavement Coordinators
Bereavement support is offered by our team to address the phases of grief and acceptance. Support is offered to families for at least one year after the patient has died. Hospice Compassus also helps with funeral arrangements and can help guide family members through other issues necessary to address after death.
Volunteers
Our compassionate and highly trained volunteers are giving of their time and talents and help in many ways from offering companionship, grief counseling to running errands.
Your Physician
We encourage and include your personal physician in our team approach and in planning your care. Patients can choose to have their doctor involved in their care. Your physician will work in collaboration with our Medical Director to assure patients receive excellent care and continuity.
