Healthcare Providers
Hospice Care of the Southwest sets itself apart from other hospices by delivering high quality compassionate care in a timely manner. We are able to do this by hiring the most qualified employees and by continually training employees in patient care. We also set ourselves apart by utilizing point-of-care technology to enable the delivery of real time high quality information throughout our care continuum. We lead the hospice industry with our innovative medical record technology. In other words, our clinical staff have clinical notes and medical histories of patients at the bedside. Physicians
We know that many physicians like to participate in the development of their hospice patients' Plan of Care and to be kept informed of any changes in their patients' condition. We are a "physician-friendly" hospice and encourage referring physician participation in the care that we offer to your patients. We will actively communicate with you about your patients' condition using the communication protocols we establish with your office. We encourage you to try our service and see how it is different from other hospice providers.
Long-Term Care Providers
Hospice Care of the Southwest's Long-Term Care "service promises" are custom designed to support your facility's ongoing efforts to provide the care that terminally ill residents need and are entitled to as part of their Medicare and Medicaid Benefits. We know how important it is for you to provide continuity of care for your residents while being fiscally responsible in your operations. We help you keep residents in your facility – through crisis care when appropriate and with the supplemental resources provided under the Medicare Hospice Benefit – such as visits from members of the hospice team, as needed by the patients – any day of the week. We can also contract with your facility to provide the hospice inpatient level of care for patients that meet the criteria for that level of care – if your facility meets the regulatory requirements.
Patients & Family
An innovative concept in healthcare, Hospice brings together a team of specially trained professionals and volunteers who work with the patient's doctor to provide a plan of care designed to control the pain and ease end-of-life struggles for the patient and family.

The Hospice Team is made up of professionals from a variety of disciplines and specially trained volunteers working under the stewardship of a Hospice medical director to meet the individual needs of terminally ill patients and their families. The Hospice Team also works with the patient and family's community of faith and/or other spiritual care resources, as requested.
For residents of a long-term care facility, Hospice offers a special program that works in concert with the facility staff to provide terminally ill patients with state-of-the-art symptom management and emotional and spiritual support, for themselves and their families, during this time.
