Defining Home Care Services
People are often confused about the meaning of the term “home care services” so let’s make sure we clear up this confusion.
Individuals are able to receive a variety of health and personal care services in the convenience of their home. The goal of home health care is to provide a course of treatment for a specific illness or injury. Home health care is medical care designed to help you get better, regain your independence, and become as self-sufficient as possible as quickly as possible.
Home care (more typically referred to as personal care, private care or companion care) is considered non-medical care. The objective of home care is to provide assistance for an individual so that he/she can continue to live in their home and enjoy their independence to the fullest extent possible.
Companion Care Services
Numerous surveys confirm that a large majority of people age 60 and over desire to live out their years in their own place of residence. In order to do this many people as they age need assistance with performing and completing various tasks. These “activities of daily living” refers to a set of common, everyday tasks, the performance of which is required for personal self-care and independent living. The commonly recognized six activities of daily living are:
- Eating (the ability to put food into the body).
- Bathing (the ability to adequately wash oneself in a bathtub or shower, including the ability to into and out of a bathtub or shower).
- Dressing (the ability to put on and take off clothing, including jewelry and accessories).
- Toileting (being able to get to and from the toilet, to get on and off the toilet, and to perform associated personal hygiene after using the toilet).
- Transferring (the ability to move in and out of chairs, beds, wheelchairs, etc.).
- Maintaining continence (the ability to control one’s bowel and bladder functions and perform associated personal hygiene).
In addition to the standard list of activities of daily living, individuals may also need assistance with other types of activities that most people periodically need to perform. Examples of these activities include the following:
- Cleaning the home.
- Doing laundry.
- Driving to run errands and handle personal business affairs.
- Cooking meals.
- Managing multiple medications on a daily basis.
- Shopping for food, clothing and other basic necessities.
- Performing lawn and garden maintenance.
Companion care service is the answer for those that need support or assistance with any of the activities of daily living or other associated activities of living life. From meal preparation to bath and dressing assistance to running errands or helping with trips to the doctor’s office, companion care service can be arranged to suit every client’s specific needs. We serve clients as little as four hours one day a week all the way to 24/7 companion care where a Horizons caregiver is always with the client.
Every client has different needs and wants. Using our Solutions for Living© approach to care, we utilize our free in-home consultation to listen to our clients and their families about their specific circumstances. After we fully understand the needs of the client, we craft a client specific Plan of Care. Hospital Companion Care
Hospital stays, even if for just one night, can be unnerving and stressful for many people. Having someone looking after your best interests just might make that hospital stay less stressful. A Horizons professional caregiver can be scheduled to stay in your hospital room with you for part of or the entire duration of your hospital stay. And if needed, your Horizons caregiver can go home with you to assist with your recovery.
Bath Service
Let us help you or your loved one that needs assistance with bathing. We can design a schedule to fit virtually any need. A skilled Horizons caregiver will provide complete assistance with the bath from start to finish.
Medication Management
Many individuals need assistance in setting up and taking their medications per the instructions given by their physician. Horizons can assist with setting up medications on a regular schedule so that they can be taken per the prescription requirements. We can also provide routine medication reminders for those that may need occasional reminders about taking their medication.
Check-on Service
This service is well liked by many families who live elsewhere but have a loved one in our service area. We can tailor this service to fit specific requirements, from check-on telephone calls to scheduled in-home visits. This service also works well for local folks who want their loved one checked on while family members are traveling and not available.
Specialty Nursing Service
Horizons staff of registered nurses (RN’s) and licensed practical nurses (LPN’s) can help fill the void when assistance is needed in handling minor medical issues that don’t dedicate the use of a licensed home health care provider. We have performed a variety of tasks for clients over the years, from changing wound dressing on a specific schedule to giving a series of shots to a client over a several day period.
Respite Care
The demands of being a caregiver for a friend or family member are significant. Caregiving is simply put, stressful work. Respite care is short-term care given to a patient by a different caregiver, so that a family member or friend who is the patient’s regular caregiver can rest and have some time off. The goal is to provide the regular caregiver with a break while at the same time continuing to meet the care requirements of the individual receiving care.
Every caregiver needs a break periodically to take care of themselves and that’s where Horizons can help. Let our team of professional caregivers fill the gap of care so that your caregiver can have a much needed break. We can arrange to provide respite care on any schedule, from a scheduled once a week break for a several hours to taking over care on a 24/7 basis to allow for a break covering several days.
Hospice Continuous Care Staffing
Horizons provides continuous care staffing services to hospices in our market area. Our staff includes a number of hospice trained and experienced RN’s, LPN’s and CNA’s.
Hospice and Facility Emergency Staffing
Horizons provides emergency staffing assistance to several hospices and senior care facilities in our area. With our team of skilled and experienced nurses and CNA’s, we can fill any need quickly and efficiently, from a single nurse for one shift to several nurses and/or CNA’s to cover vacations or any type of emergency circumstance.
