Home Health Connection is based out of Charlotte, North Carolina. We provide comprehensive family centered home health care to residents of Mecklenburg, Cabarrus, Gaston, Union, Rowan, Iredell, Guilford, Durham, Surry, Yadkin, Forsythe, and Davidson Counties. Licensed by the state of North Carolina as a home health agency, we are also CARF accredited. Our mission is to provide service with dignity and respect.

Home Care Services:

• Hygiene Assistance
• Incontinence care
• Help with medications
• Impaired Mobility Assistance

• Light Housekeeping
• Errands and Shopping
• Doctor Appointments
• Companionship

Nursing Services:

• Registered Nurse (RN): assessments and observations
• Licensed Practical Nurse (LPN): provide all aspects of patient care under supervision of RN
• Certified Nurse’s Aide (CNA): provide assistance with physical care and activities of daily living
• Companion Sitting/Private Pay: provides emotional support and comfort
