Home health care is an extension of your doctor’s care and hospital services. It is provided to you in your home. Care may include an array of therapeutic and support services to give the patient maximum independence in the home.
Who qualifies for Home Health under Medicare?
To qualify for home health under Medicare, you must be home-bound, under the care of a physician, and in need of skilled services on an intermittent basis. Intermittent home care services are those that are only needed a few hours a day.
You do not have to be bed-ridden or totally confined to home to qualify for Medicare reimbursement. You are considered home-bound if you are unable to leave home without considerable and taxing effort on an infrequent basis.

Highlands Home Health develops a plan of care with your physician, and the plan of care is closely supervised by our clinical staff. A registered nurse visits your home and develops a plan of care with your physician. The care plan is supervised by your case manager (a registered nurse). All nurses, therapists and other staff stay in close communication with each other and with your doctor and notify the doctor of any changes in your condition or needs.
