At Heartland, we help preserve our patients’ dignity, show them the respect they richly deserve and provide caring compassion by making these Heartland Caring Promises and by working hard to keep them.
Heartland Hospice is dedicated to providing patients with care wherever they consider home. In the house they’ve owned for years. In a nursing home. Or in an assisted living facility. Hospice is caregiving tailored to fit the patient’s and family’s unique situation. Hospice supports the patient and family wherever they are in their health care journey and upholds the patient’s and family’s wishes, goals and values. Patients need not lose hope nor stop comforting treatments to receive Heartland Hospice services. Advance directives and health care decisions do not need to be completed before receiving hospice care. We encourage and can facilitate open discussions that foster the understanding of a loved one’s health care choices, while supporting the patient and family during these challenging circumstances.

We build a unique partnership with patients and their loved ones, which supports our patients’ and families’ goals and encourages families to be involved in all care decisions. Heartland’s focus is on sustaining quality of life by encouraging personal choices, supporting meaningful experiences and preserving patient dignity.
Accessing our hospice care is simple. We meet patient needs and make it easy for everyone to get the care to which they are entitled. Hospice care is available to patients with an advanced illness. Anyone can call Heartland Hospice to receive information about hospice services. We offer support to patients and families 24 hours a day, seven days a week. Heartland will also contact your physician to ensure that the patient is eligible for the hospice benefit.
Heartland Hospice provides specialized care that meets the physical, emotional and spiritual needs of patients and loved ones facing the challenges of an advanced illness. The quality of life for those we serve is enhanced by encouraging personal choices, supporting meaningful experiences and preserving dignity.

We offer hospice services wherever a patient calls home, whether the care is provided in a home setting, assisted living facility or skilled nursing center. In some locations, we offer Heartland Hospice House — an inpatient care center for patients who need assistance managing pain or stabilizing their condition before returning home.
Circle of Care Defines Our Culture
It has almost become a cliché to hear a service provider tell you how much it cares about its patients and meeting their needs. What defines HCR ManorCare’s commitment to caring is 20 years of comprehensive training to bring out the helping, respectful and responsive nature that is within our employees. This training is the company’s Circle of Care® program. More than a program, Circle of Care is a philosophy, a philosophy focusing on how we should treat one another.
Circle of Care uses a variety of learning methods that affect an employee’s senses of sight, hearing and touch. Classes, group discussions, videos and role-playing activities help employees enhance their caring attitude not only toward patients, but to patients’ families, vendors and other employees, as well. The essence of Circle of Care is taking the time to listen to, understand and help everyone with whom you come into contact. It is taking a personal interest and “exceeding the circle” — going beyond expectations — to meet someone’s needs.
The program’s modules teach people how to listen, to say the right thing at the right time, to understand and effectively use body language, to understand what motivates the actions of families, and to help ensure patients and their families are satisfied guests. One of the most important modules focuses on improving employees’ self-esteem, because we have learned when we feel good about ourselves, those feelings transition to how we feel about and care for others.
Working in the health care environment can be very demanding, and Circle of Care has proven to be a valuable life experience that shows our employees the importance of caring, having pride in what they do and always putting forward their best effort.
