At ElderCare 4 Families, we stress the importance of working as a team with our clients to provide the best home health care possible and use the following solutions to achieve these results.
1. Graceful Aging Program (GAP). Ensuring our clients are receiving the best home health care possible is our highest priority and we achieve that goal through our Graceful Aging Program. This program is designed to make certain the best care is established from the beginning and continued each and every day through the supervision of our Senior Advocates. The Senior Advocate conducts the initial assessment prior to starting care. All the information gathered from the assessment is developed into a Care Plan and it is updated by the caregiver during every shift. Based on the needs of each client, the Senior Advocate routinely visits to make sure the wishes of our clients are always being met.
2. Safe4Hire. This program is unlike any other in our industry. We go above and beyond the competition to ensure we hire only professional, ethical and dependable caregivers. This process involves prescreenings, applications, interviews, reference verification, nurse abuse registry checks, motor vehicle record reviews, criminal background checks, drug screenings, social security checks, orientation, ongoing training and continuous monitoring by our Senior Advocates.
3. Accountability. Our caregivers are required to clock in and out through a monitored telephone system called Telephony. Once the caregiver has clocked in, the Telephony system calls back to verify their location. This ensures billing and payroll are accurate and all shifts are covered. If a caregiver has not clocked in, the Telephony system will notify our staff to cover this shift, no matter what time of day or night.
4. Matching Clients with Caregivers. It is important that each client is staffed with the best caregiver to fit their needs. We want to build trusting relationships with our clients. We carefully evaluate our caregivers’ skills and strengths when matching them with clients, and continue to monitor the relationship to ensure it remains a good fit.
