About Us
Our History
EHC Hospice otherwise known as East Harris County Hospice Services has been serving patients in Houston and surrounding areas since 1995. We are a Medicare/Medicaid certified hospice agency licensed by the Texas Department of Aging and Disability. We are also members of the National Hospice and Palliative Care Organization. Our commitment is to the patients and families we serve each day, ensuring that all aspects of a holistic approach are fulfilled.

Our Team
We have a very dedicated and committed team of health care professionals known as the Interdisciplinary Team, who share our mission of ensuring that each patient and family be treated with respect and dignity. In accomplishing that, we promise to meet each need on a prompt and personal basis ensuring all aspects of care are met.
The Medical Community
EHC Hospice has been instrumental in extending palliative care & support to hundreds of patients and their families in their end of life journey. We have worked as a team with many area physicians, hospitals, and long term care facilities in providing the individualized care required by patients and families.
Services
Our team provides services to terminally ill patients and their families meeting their physical, emotional, social and spiritual needs. Our focus is on enhancing the patient's quality of life fully with dignity & comfort at the patient's home, a nursing home, assisted living facility, or a personal care home.

Our offices are open from 8:30 - 5:00 pm Monday through Friday. However, our services are available 24 hours a day, 7 days a week, including holidays, with a nurse on call to attend to cares and concerns patients and families may have.
At EHC Hospice, care is provided by a physician directed group of interdisciplinary team members from different professions and expertise.
EHC Hospice's Team Includes, but not limited to:

- Certified Nurse Aides/Home Health Aides
- Pastoral/Spiritual Support Counselors

- Ancillary service providers for durable medical equipment, pharmaceuticals, & ambulance transport
Levels of Hospice Benefits
Routine
Palliative care is provided on an intermittent visit basis. Nurses and home health aides work as a team to insure the patient and the family is supported during this special time.

Inpatient
Palliative care is provided in a Long Term Care Facility (LTC) or an acute care hospital. The facility will have a Registered Nurse on duty 24 hours a day and is used for symptom management. The EHC RN will take the role of Case Manager and ensure that the patient is symptom free during the inpatient stay.
Respite
Palliative care is provided in a Long Term Care Facility (LTC). The facility will have a Registered Nurse on duty 24 hours a day and is used for symptom management. The EHC RN will take the role of Case Manager and ensure that the patient is symptom free during the inpatient stay.
Continuous Care
EHC Hospice nurses and home health aides will provide palliative care in the patient's home for longer periods of time, any hour of the day and night. There is a diagnosis specific medical criterion that must be met before Medicare will authorize Continuous Care.
