About Us

Covenant means a ‘promise’ or ‘pledge’. The ‘Covenant’ in Covenant Companies represents our commitment to provide our clients with the professional care they value from their health care provider.

Our Coon Rapids headquarters houses a caring, capable, and motivated staff with a heart for serving and a ‘can do’ attitude. We have over 10 years of experience serving the senior community in the north suburbs of Minneapolis with skilled intermittent care including:

• nurses
• personal care assistants
• home health aides
• homemakers
• physical therapist access

We are Class A Minnesota licensed and Medicare certified to administer home health care. Our goal is to help you achieve independence as quickly as possible. We want to keep you in your home as long as possible, so you can maintain a great quality of life throughout your changing health condition.

Personal Care

Covenant Care’s personal care assistants provide necessary services to clients as determined by the patient’s plan of care. Our personal care assistants are under the direct supervision of our supervisory nurses. Some services provided include:

• Basic personal care such as toilet and grooming activities, bathing, hair care, assistance with clothing or exercise
• In-home assistance with medications/health care.
• Assistance with nutrition such as menu planning, grocery shopping and meal preparation.
• Documenting behavior, activities and progress on data sheets; completes various reports and forms
• Evaluates and determines individual's strengths and needs

Home Health Aides

Covenant Home Care’s home health aides provide health-related services for patients under the directive of a licensed RN. Home health care aides provide services like:

• Administering oral medications or medication reminders
• Checking pulse rates, temperatures, and respiration rates
• Helping with simple prescribed exercises
• Keeping client’s room clean
• Moving patients from beds/chairs
• Bathing
• Dressing
• Grooming
• Changing nonsterile dressings
• Giving massages
• Assisting with braces and artificial limbs
• Preparing meals

Covenant Home Care’s home health aides keep stringent record of services performed and record their client’s changing health condition. Changes are reporting to registered supervisory nurses, and the care plan is reviewed at least monthly to determined if changes need to be made in the care plan.
