Clare Bridge of Plymouth located in Plymouth, Minnesota provides Alzheimer's and dementia care for seniors and offers the promise of Daily Moments of Success. Highly Individualized Care

Maintaining familiar daily routines is important for our Alzheimer's, dementia and memory care residents. Physical and behavioral needs, as well as life history each play integral roles. Each resident receives and individualized service plan designed to meet their unique needs and to help them feel safe while adding meaning to their day.

Compassionate Assistance

Individuals working with Alzheimer's and dementia residents must have compassion and understanding. Our staff are carefully selected and trained to manage the challenges associated with dementia, which can sometimes be difficult. Personalized Service Assessment and Plan

Our personalized assessment evaluates individual needs and preferences. Then we customize a service plan to meet those needs and preferences. The personalized plan is based upon the amount of time it takes to provide quality services. Therefore, residents only pay for what they need and want.

Ongoing dementia training, expertise and support for staff

Our staff is trained to understand and manage unpredictable behaviors and the need for an individualized approach for every resident, depending on the progression of the disease. We understand that creative problem solving is needed when providing dignity and quality of life to individuals with memory loss. For this we have dedicated dementia care specialists on staff to support our staff, residents and families.

Assistance with Personal Needs

Our trained staff is there to assist with personal needs, in the privacy of the resident’s suite/apartment. We provide assistance with – Bathing or showering, selection of clothing, dressing assistance, grooming, hygiene, mobility and transferring and dining assistance.

Ambulation & Escort Services

We will escort residents to meals and activities within the building.

Three nutritious and appetizing meals served daily

We serve breakfast, lunch and dinner daily. All menu options are selected by a registered dietician to optimize nutritional value. Understanding that many individuals with memory loss have special dietary needs, we offer a full menu of finger foods and adaptive devices and document food preferences for every resident. Resident seating is pre-selected to insure a pleasurable dining experience.

Snacks are served throughout the day

Housekeeping Services

We clean every suite/apartment regularly and take out the trash when needed.

Personal Laundry Services

We launder personal items, linens and towels and change bed linens weekly.

Continence Management

We offer a comprehensive continence management program – from reminders to scheduling to assistance.

Nursing Services

Professional nurses oversee the delivery of all health care services and behavior management to our residents. Our nurses are on call 24 hours a day, seven days a week.

Ongoing monitoring of residents’ health status and tracking of behaviors

Medication Support

Our staff provides comprehensive medication support – from medication reminders to standby assistance to administering medications.

Life Enrichment Programming

Prior to moving in, staff take a social history on every resident to get to know their individual likes and dislikes. Programs and outings are specially designed with residents’ individual interests and abilities in mind. Multiple programs are offered throughout the day and evening hours. These programs are designed and supported by our dementia care specialists.

Specialized intervention programs

We know that good monitoring and intervention in specific areas can help avoid illness and/or injury and improve quality of life for our residents. For this we have developed and implemented specially designed nutrition tracking, weight management, behavior evaluations and falls management programs.

Short term stay

Our short term stay program is available for family vacations or just a much needed rest for family caregivers.

Ancillary Services

We offer additional services for unscheduled needs and personal requests – i.e. room service, escorts to outside appointments and shopping, running errands, etc.

Specially designed environment

Our community offers a home like environment, which is ideal for individuals with memory loss. Our town square concept is designed to allow safe wandering, reduce agitation and promote appropriate stimulation. Our purpose built community incorporates special features, including circular paths designed to lead residents to areas of activity, enclosed, outdoor courtyards, small spaces decorated like home and shadow boxes for residents personal mementos to identify their personal space.

Our emergency response system, complete with emergency pull cords, is monitored 24-hours-a-day, by our trained staff.

Clare Bridge offers nutritious, restaurant-style meals served by our trained wait staff, and our chefs are masters at creating traditional meal favorites plus a selection of fresh, flavorful dishes using low-sodium, heart-healthy cooking techniques.

We strive to offer our residents plenty of choices in their dining experience. They choose where to sit and what they want to eat. The dining room has extended hours for residents’ convenience.

Meals are prepared by our trained chefs, using only hand-selected produce, the freshest herbs and spices, and the freshest meat and seafood. Clare Bridge's chefs will gladly cater special events, even preparing dishes from residents’ own recipes. A private dining room is available to entertain family and friends.

Resident comments and menu suggestions are reviewed daily and are used to tailor menu selections.
