Our History
Christiana Care Visiting Nurse Association has been providing an impressive range of home and community-based health services since 1922. We're the largest and oldest statewide home health care organization in Delaware. Chronic Disease Management
You or your loved one can remain independent and healthy at home, and the Christiana Care VNA can help. And because we keep your doctor updated on your progress, you will gain added peace of mind that your health care team is working in a coordinated manner to ensure your best health.
Home Rehabilitative Services
The Visiting Nurse Association's rehabilitative team can provide you with the full range of rehabilitative services that you need to recover at home. Your very first visit from our rehab team will include a thorough evaluation of your home and your functional ability. Based on this assessment and under the direction of your doctor, we will establish an individual care plan that will set you on your optimal road to recovery.
Medication Management
Poor medication management is one of the main causes of patients being readmitted to the hospital after initial discharge. Taking the wrong dosage, not understanding how to properly take medications, improperly stopping medications, and mixing incompatible prescription and over-the-counter drugs—these are just a few common mistakes that can put your health or the health of a loved one in jeopardy.
Mother and Baby Home Care
Mother and baby home-care visits are only available in New Castle County.
Caring for a newborn baby can be challenging, but when your baby is premature or at high risk, the experienced help of a visiting nurse can be invaluable. We specialize in caring for premature or high-risk infants. Working with your doctors, our experienced registered nurses can coordinate your care and monitor the health of you and your baby at home. We strive to prevent or reduce rehospitalization, to maximize good health and to promote appropriate growth and development.
Your care may require several home visits, especially during the first several weeks. Parents and caregivers are often sleep-deprived during this time and need additional instruction and support with their tiny infants. Your VNA nurse can ensure good communication and coordination of your health care team, which is especially important during this time.
Private Duty Nursing and Pediatric Home Care
Christiana Care VNA's private-duty nursing is trusted by more families throughout Delaware to provide the highest skilled care to children with the most challenging pediatric health problems. Alzheimer's and Dementia Day Care
In addition to the VNA at-home services available for patients with Alzheimer's disease and dementia, the Visiting Nurse Association's Evergreen Alzheimer's Day Center provides care for people with Alzheimer's and related dementias, and support for their caregivers. Our adult day care provides a therapeutic program with activities designed to provide social skills, reduce stress and encourage self-confidence. And it provides caregivers with valuable time to work, de-stress or pursue other activities.

The Christiana Care Visiting Nurse Association provides skilled nursing care, home health aides, physical therapy, occupational therapy, speech therapy and medical social workers—all backed by the Christiana Care name that you trust. Our specialized nursing programs include: cardiac; diabetes; cancer care with a focus on pain and symptom management, treatment and support; wound care; infusion therapy; telehealth; and dementia care. VNA clinicians use state-of-the-art technology, including digital photography to assess wound management, mobile computing and telehealth for cardiac and respiratory monitoring.
Is the provider certified by the Joint Commission?
The Joint Commission's Home Care Accreditation Program is the gold standard in home health care accreditation. VNA has a vigorous quality-control program meeting Joint Commission standards and an active performance-improvement committee that identifies goals and areas of focus and reports to the VNA board.
How often does the provider communicate with my doctor?
VNA professionals communicate with physicians initially at the start of service or when there are changes in the patient's condition. We speak to the physician to modify the plan of care.
What procedures are in place to handle emergencies?
We teach patients and caregivers what to do in the event of an emergency, and we develop an emergency plan with them. Members of our clinical staff are on call for phone advice at all times.
Are the agency caregivers available 24 hours a day?

Are services available for extended hours of care?
The VNA has private-duty nursing and home-health-aide services available for anyone who would like to privately pay for services that are not covered under their insurance policy.
