Cedar Lake Residences
CLR offers a variety of community-based residential housing and support services to more than 100 clients.
About Us
In the late 1980s, when community integration was not being practiced, Cedar Lake Residences, (CLR) became a leader in assisting persons with intellectual to mainstream into the community. CLR offers a variety of community-based residential housing and support services to more than 100 clients. CLR's options include 24-hour staffed residences and group home, transitional living, in-home services, supported living apartments and HUD housing. CLR properties (single-family and patio homes, as well as apartments) are located in Jefferson, Oldham and Henry counties.
In late 2006, the adult day health program moved from the Lodge to a community facility nearby in LaGrange, and became a part of CLR. Currently, more than 50 active clients are served at the Cedar Lake Enrichment Center.
Services
Cedar Lake Residences, a subsidiary of Cedar Lake, Inc., is a private not-for profit corporation which provides community-based residential options and support services for adults with intellectual and developmental disabilities. Cedar Lake has provided services to persons with MR/DD since 1974, and began providing community-based services in 1989. Cedar Lake
Residences began distinct operations with independent incorporation in 1993. Our service options include a licensed group home and several staffed residences providing 24 hour support, a 24 hour staffed apartment building, patio homes with 12 hour a day staff support, independent living apartments with intermittent support, in-home and case management services. Each resident has an individual, annual Plan of Care, which includes goals of his/her choosing to work on throughout the year. Residents are encouraged to make "personal choice" a major part of that plan and of their daily life. Case managers coordinate the Plan of Care to support the goals through whatever services may be required or requested.
Group Home
This community residence opened in February 1989. Its gracious style blends well into the LaGrange Community.
Typically for individuals with mild or moderate mental retardation, this option is a semi-independent, twenty-four (24) hour-a-day staffed home for six residents. The residents work or volunteer in the community 20 or more hours per week. Residents receive training and support in such areas as daily living skills, transportation, leisure and recreational activities, as well as benefits coordination.
Apartments
Supported Independent Living Apartments in LaGrange and Louisville, which are owned by CLR. (Most with HUD funding)
Residences-Services Apartments 1Residences-Services Apartments 2This program provides supported independent living options for persons with mild or moderate levels of intellectual and developmental disabilities who desire to live independently in the community. Cedar Lake staff facilitators visit their home a minimum of twice per week to work on areas of need specified in their plan of care. Individuals are encouraged to participate in community employment, day program or volunteer opportunities at least part time during each week. Clients typically receive supports in areas such as nutrition planning, grocery shopping, financial management, developing social skills, homemaking and safety skills as well as benefits coordination.
Jefferson Manor
This option consists of two three bedroom patio homes which are staffed twelve (12) hours a day. It is designed to provide a level of support between the group home and independent living. The six residents are encouraged to participate in the community at least part time during the week.
Staffed Residence
This option provides 24-hour care for three individuals in a single-family home setting, smaller than the group home. Residents enjoy a variety of opportunities and choices for competitive and small business settings employment, day programs, cultural and recreational activities, even vacations.
In-home Supports
This option is designed to provide supported living services to persons who reside in their own or family homes.
Service plans are developed on an individualized basis to meet client and family needs, and to encourage independence.
CLEC
The Cedar Lake Enrichment Center houses the adult day health program of Cedar Lake Residences. The adult day program originally started on site at Cedar Lake Lodge in 2002, but moved to a community facility located in LaGrange in November 2006 and became part of CLR. The center currently serves more than 30 active clients, with staff to client ratio at 1 to 5.
CLEC offers a medical model in adult day health services, and serves clients from the Lodge, CLR and the community at large. CLEC has a nurse on staff and can serve both very high functioning individuals with mental retardation, as well as those who are medically fragile.
