Care For Life is a Geriatric Care Management agency, locally owned and operated by medical professionals that provides home care services for the aged for 4 to 24 hours, seven days per week. Our compassionate caregivers provide personalized care to each client in accordance with a detailed care plan established by the RN Clinical Director following an in-home comprehensive health and safety assessment.

All caregivers have background and reference checks, are bonded, insured, trained and supervised by Care For Life’s Quality Assurance Managers and the RN Clinical Director. We offer 24/7 on-call services for our caregivers and clients. Caregivers are monitored by unannounced Quality Assurance visits by a Care Coordinator with decades of care giving experience.

Care Management

What sets Care For Life’s Home Care apart is ongoing Geriatric Care Management. It provides invaluable peace of mind for adult children who live in other cities who want a skilled Care Manager checking in on their parent whether at home or in a facility in the Charleston tri-county area. Care For Life becomes the children’s “eyes and ears” and correspond by telephone or email with the adult children.

The Care Plan

The in-home assessment by the RN Clinical Director is required prior to beginning care. The resulting written care plan establishes what caregivers will do for the client. Assessment of the home environment, family dynamics, the client’s physical and mental health, self-care abilities and whether there are undiagnosed medical problems are all points to consider in planning care.
