San Marco Terrace, owned and operated by Brooks Rehabilitation, is an award winning nursing and rehabilitation facility located in the heart of St. Augustine. One of only a few facilities in NE Florida awarded the 5 Star Quality Rating by Medicare, San Marco Terrace is among the top 10% of all skilled nursing and rehabilitation facilities in the United States

Brooks Rehabilitation has been serving Northeast and Central Florida for over 35 years. As a non-profit organization, Brooks Rehabilitation, based in Jacksonville, FL, operates the second largest inpatient rehabilitation hospital in the US, with 157-beds, one of the region’s largest home healthcare agencies Brooks Home Care Advantage, 26 outpatient rehabilitation clinics, and an award winning skilled nursing facility. In addition, Brooks Rehabilitation is conducting over 20 clinical research trials to advance the science of rehabilitation.

The Brooks Rehabilitation Hospital, along with our Spinal Cord Injury, Pain, Stroke Recovery and Brain Injury programs, is accredited by the Commission on Accreditation of Rehabilitation Facilities (CARF).  Brooks is also accredited by the Joint Commission and listed as a state designated facility by the Florida Brain and Spinal Cord Injury Program (BSCIP).

San Marco Terrace is proud to offer Physical,Occupational and Speech Therapies to our residents
who need these services.  Therapy takes place in a variety of settings throughout our campus. In addition to our therapy gym, residents may learn techniques for navigating different surfaces in our outdoor therapy courtyard, A therapist may have a resident practice walking down a hallway while greeting other residents.

Brooks Rehabilitation is a not-for-profit organization with a mission to advance the health and well-being of persons requiring rehabilitation through superior outcomes, service, education and research.Brooks Rehabilitation skilled nursing services feature inpatient nursing and rehabilitative services to patients who require continuous care. While at Brooks, you are under the care of a physician and a licensed nurse.

Therapies woven throughout the day during a resident’s stay will ensure continued success
as they return home or to another level of care.  Our therapists and professional staff are committed to helping each client maintain quality of life, independence and dignity while achieving their highest level of recovery. We realize that each resident has unique needs and challenges. Our  staff will develop an equally unique, comprehensive rehabilitation program.

Total Joint Rehabilitation

Two of Jacksonville's leading healthcare providers  -- St. Vincent's HealthCare and Brooks Health System -- have come together to offer this unique skilled nursing unit located within St. Luke's Hospital. The innovative sub-acute rehabilitation program, the first of its kind in Jacksonville, is designed serve the needs of the community and is available for for patients at all area hospitals.

Our ultimate goal is for every patient to return home safely and to recover successfully.

Highly specialized experts

All therapy is administered by highly specialized experts with advanced training in orthopedic rehabilitation.  We also have a team of specially trained nurses on hand to provide around-the-clock nursing care.  The program is guided by a physician with board certification in physical medicine and rehabilitation.  The program is directed by a Director of Nursing.

Stay connected to family and friends

Our newly renovated wing is fully wired to allow patients to stay in touch with loved ones.  We offer access to lightweight laptops.  Patients and visitors can also watch favorite movies using our Netflix account.

Rehabilitation seven days a week

We offer a comprehensive range of programs throughout the week and on weekends.  Our goal is to facilitate every aspect of your recovery and transition back to daily life.  Your rehabilitation includes:

•     Physical therapy- to build strength while reducing pain
•     Occupational therapy- to increase independence with daily activities
•     Pain management strategies to reduce discomfort
•     Caregiver Education
•     Activity therapy to integrate rehabilitation goals with leisure lifestyle choices
•     Trained Therapy Dogs to provide friendly encouragement

Powerful tools for recovery

Upon admission, each patient is given access to DVD/video technology, including the Wii Fit that will enhance the therapy experience and allow you to more easily continue your exercise program at home.  Our dedicated gym space is filled with state-of-the-art equipment and we also offer a simulated kitchen and laundry to teach patients adaptive techniques for a safe and functional return home.  You will be provided with the services of a case manager to coordinate your rehabilitation and help plan your continued care after discharge.
