Our nursing and rehabilitation center offers short-term rehabilitative as well as long-term care - a full range of medical, nursing, rehabilitation and social services to treat and support each of our patients and residents. Many of our patients go home after a few weeks and, for those who are unable to return home right away, we provide safe, compassionate care in an environment that fosters independence and dignity

When patients leave the hospital, they often need continued care to recover completely. That’s where we come in. We work with you to set a care plan to meet your goals and abilities. Patients typically receive up to three hours of intensive rehabilitation each day, provided by our specially trained interdisciplinary team.

Our team treats a variety of conditions including:

post-stroke recovery
hip fracture/replacement
knee or other joint replacement
neurological illnesses (Parkinson’s disease, etc.)
cardiac or respiratory ailments
tracheotomy weaning
post acute COPD episode
post ventilator care
post open heart (grafts, valves, stints)

Long-term care is when a person requires assistance with his or her physical or emotional needs over an extended period of time. The need might be due to a terminal condition, disability, illness, injury or the infirmity of old age. The need for long-term care may only last for a few weeks or months, or it may go on for years.

We provide the services and supplies that your family member needs while he or she is a resident at our center. This help may be required for many of the activities or needs of daily living, including:

walking
bathing
dressing
using the bathroom
managing pain
preventing unsafe behavior
preventing wandering
assistance with eating
personal hygiene and grooming
administering medications

Keeping our patients and residents involved in interesting and varied activities and events is one more way we promote healing and recovery.

We have a staff of certified activities professionals who create and coordinate activities at our center. Activities are tailored to a resident’s and patient’s individual interests and abilities and include:

outdoor pursuits
entertainment and audience participation
games and other activities to sharpen mental abilities
spiritual enhancement and reflection
