Skilled Nursing
Aston Home Health's Registered Nurses and License Practical Nurses provide care to patients at the comfort of their home.
Treatments given to patients include, but are not limited to diabetes management, respiratory care, wound care, tracheostomy and colostomy care, as well as injections and other medication administration.
Our patient's health condition is managed by a specific health care team. The nurse, together with all disciplines involved, construct a plan of care in order to meet the patient's treatment and recovery at the soonest possible time. The plan of care is regularly communicated to the patient's referring physician.
Our nurses are available 24 hours a day, 7 days a week. We have on-call nurses to take your emergency calls beyond office hours.

In-Home Physical Therapy
Our therapists concentrate on re-teaching patients to perform activities meaningful to them.
The goal of our in home Physical Therapy is to improve the patient's health after an illness, accident, or due to the developmental challenges of aging. Our therapists work closely with your physician, and develop a plan of care which may include physical exercises, and home/environment safety evaluation.
Our therapists also evaluate your need in obtaining durable medical equipment.
Social Services
Our social workers are capable of providing a wide range of services beyond community resources, including short-term therapy and crisis intervention.
They identify social and emotional problems impacting the patient's medical condition, treatment plan, or rate of recovery. Our social workers establish a treatment plan with goals to help patients and their families with problems such as emotional, community resource and money management.
Our social workers work hand in hand with the home health team in handling patient problems. They facilitate support groups for families of the terminally ill.

Home Health Aide
Our Aides help patients who need: Companionship, Alzheimer's & Dementia Care, Bathing & Grooming Assistance, Meal Preparation & Clean Up, Light Housekeeping, Medication Reminders, Laundry & Linen Changing, Errands, and Walking Assistance.
Our aides are specially trained to help patients perform every task made difficult by illness or injury.

Aston Home Health employs registered nurses (RNs), licensed practical nurses (LPNs), physical therapists (PTs), social workers (MSWs), and home health aides (HHAs).
Our stringent hiring standards, ongoing supervision, 7 day clinical support, and comprehensive training and education ensure that our caregivers give clients the highest quality health care services available. Aston Home Health is also accredited to ensure compliance with health care standards and all of our caregivers are fully insured.
Stringent Hiring Standards
We thoroughly verify credentials, references and experience, and perform criminal background checks on all applicants. After in-depth interviews, written testing, and hands-on skills assessments, we also evaluate their compassion, excellence, and reliability - the core values of Aston Home Health.
Ongoing Supervision
Once hired, all caregivers are supervised by a registered nurse, who makes regular home visits to ensure the health and safety of our clients. Our caregivers are also required to demonstrate their competency annually with hands-on skills assessments and performance evaluations.
24 Hour Clinical Support
Aston Home Health is committed to our clients' safety. Both our clients and our caregivers have access to 7 day clinical support. Each caregiver in the field also has a dedicated clinical manager in the office that supports them, and each clinical manager has a companywide nursing office that supports them with the latest clinical information, mentoring, and best practices.
Commitment to Education and Training
We believe in continuously improving our work through evaluation, education, and training. Our programs include a range of instruction, written exams, hands-on skills lab work, and opportunity for time in the field to perform a client's care prior to independent assignment. All caregivers also undergo annual skills evaluations and have access to in-services and scholarship opportunities.
