Our services provide a team of registered nurses, medical social workers, chaplains, home care aides, volunteers and also grief counseling.
We also provide a Hospice and Palliative Care Certified Medical Director on call 24/7.
Our staff at Affinity Home Hospice Services knows the End of Life for one family member affects the lives of all other members of the family. That is why, we offer compassionate care for the entire family and the loved one at this difficult time.
What is Hospice
Affinity Home Hospice offers holistic care for the hospice patient and their families. A team of professionals and trained volunteers provides the care given to the patients and their families. It is a form of home health service to care for people with a terminal illness who are no longer being treated for cure. Our primary focus is on living, not dying. We believe that each day should be celebrated and the journey one takes during the end of life phase can be beautiful. Our staff is committed to providing the support necessary to empower patients and their families to experience death with dignity.
When people are diagnosed with a terminal illness and have been given a limited life expectancy, they have multiple needs. Not only physical needs but emotional needs as well. Patients want to be pain free and to be free of fear. Most people want to be with the ones they love and to die with dignity. Family members also experience distress during this end of life phase for their love one. Much too often the family’s needs are overlooked and neglected. Affinity Home Hospice is committed to providing support and fulfilling the needs of both the patient and their family. Affinity Home Hospice promotes comfort, autonomy, dignity while supporting the family as well as the patient.
CRITERIA FOR ADMISSION TO HOSPICE
- Physician certifies that hospice is appropriate and patient is terminally ill with a life expectancy of six months or less.
- Patient and family accept hospice and understand that the patient is not being treated for cure.
- Caregiver and/or the patient participate in the hospice plan of care.
Our Care
Affinity Home Hospice is a comprehensive approach that is customized to each patient's individual needs. Hospice is a team that is available 24 hours a day,
7 days a week.
Levels of Care
*Routine Home Hospice Care
*Continuous Care - available on a limited basis to manage symptoms that cannot be controlled without skilled nurses.
*Inpatient Care:
- Respite Care - can be provided at a contracted facility for up to five days, on an infrequent basis (usually one time in each certification period) for relief for the caregiver.
- General Inpatient Care - is provided at a contracted facility for short periods tomanage symptoms of the terminal condition. General inpatient care is designed for comfort measures, not for diagnosing and correcting life threatening problems.
- Ambulance Transport - is for general inpatient hospice care (not respite care) and is covered by Affinity Home Hospice Services. Arrangements will be made for transport by the Affinity Home Hospice team.
*Nursing Home - hospice care is available to the nursing home residents or assisted living facilities that meet hospice criteria for admission to the program.
*Bereavement - support is provided to the family and significant others for thirteen months following the patient's death.
Services
The following services are available.
- RN Case Manager - provides care to the patient as directed by the attending physician and the hospice medical director.
- Hospice Medical Director - a physician that is part of the hospice team that participates in the plan of care for each patient
- Home Care Aide - assists with the personal care needs of the patient. A physician and the home hospice team, based on the patients' needs and regulatory requirements, determine the frequency of intermittent visits.
- Medical Social Worker - provides support to the patient and their family by offering counseling and securing community resources.
- Pastoral Care - provides spiritual and emotional and support for the patients and their families.
- Volunteers - serve the professional staff, patient and families to provide care.
- Dietary and Therapist - (physical, occupational or speech) - are covered if the hospice team determines their expertise might improve patient's quality of life.
- Pharmacist - assists the hospice team with management of symptoms. Hospice covers the cost of medications for palliation related to the terminal illness.
- Medical Supplies and Equipment - One hundred percent (100%) of the charges for medical supplies and equipment needed for the palliative (comfort)care for the patient are covered. Affinity Home Hospice is not responsible for paying the cost of equipment or supplies not approved as part of the hospice plan of care.
